Die horizontale Lidverlängerung bezeichnet eine Laxizität des Unterlides, insbesondere der tarsoligamentären Stützfunktion. Das Herabsinken des Unterlides führt oftmals zum „scleral show“, was fälschlicherweise als Unterlidretraktion gedeutet werden kann. Das vorherrschende Problem der horizontalen Lidlaxizität wird dabei oftmals übersehen, was wiederum zu einer falschen Behandlung führt. Für die Korrektur dieser tarsoligamentären Stützfunktion existiert eine Vielzahl unterschiedlicher Verfahren, die auf eine Straffung beziehungsweise Neuformung des lateralen Kanthus abzielen. Zwei bewährte Techniken sollen das strategische Vorgehen in der Behandlung der horizontalen Lidverlängerung aufzeigen.
