Der spektakuläre Fall: Mitralklappenersatz bei Porzellanaorta

Einleitung

Verkalkungen in der Aorta ascendens sind wahre Herausforderung für Herzchirurgen. Die operative Behandlung solcher Patienten ist sehr schwierig, nicht genau definiert und komplex. Die größte Gefahr ist eine intraoperative Atheroembolie.

Die schwere zirkuläre kalzifizierende Atherosklerose der Aorta ascendens („Porzellanaorta”) erhöht das operative Risiko eines herzchirurgischen Eingriffs deutlich. Gründe dafür sind:

Die erschwerte intraoperative Handhabung einer solchen Aorta zwingt zu Änderungen der Operationstechnik. Das Risiko für postoperative neurologische Komplikationen infolge intraoperativ stattgehabter Atheroembolien ist erhöht.
