Eosinophile Zystitis als Symptom eines idiopathischen hypereosinophilen Syndroms

Zusammenfassung

Einleitung: Die eosinophile Zystitis als Symptom eines idiopathischen hypereosinophilen Syndroms ist ein äußerst seltenes Krankheitsbild mit schwer einschätzbarem Verlauf. Eine Kombination beider Entitäten wurde in der Literatur bisher nicht beschrieben. Durch die myeloproliferative Erkrankung kommt es zu einer abnorm gesteigerten Bildung von Eosinophilen im Blut, die ihrerseits zu Organkomplikationen führen können. In der onkologischen Literatur findet man Angaben zu kardiovaskulären, thrombembolischen und pulmonalen Komplikationen. Kasuistik: Wir beschreiben den Fall eines 2 Jahre und 4 Monate alten Mädchens, die im Rahmen einer eosinophilen Zystitis massive intraluminale Raumforderungen der Harnblase entwickelte. Schlussfolgerung: Da es aufgrund der Seltenheit des Krankheitsbildes keine standardisierten Behandlungsmethoden gibt, sollte unbedingt engmaschig onkologisch kontrolliert und individuell therapiert werden. Ziel sollte die Senkung der u. U. exzessiven Eosinophilie sein, um Organmanifestationen möglichst zu vermeiden. Insgesamt bleibt der Verlauf jedoch unberechenbar.

Abstract

Introduction: Eosinophilic cystitis as a symptom of an idiopathic hypereosinophilice syndrome is a very rare clinical picture with an unpredictable course. A combination of both symptoms has not been described previously in the peerreviewed literature. As a result of this myeloproliferative disease there is a pathologically increased production of eosinophiles in the blodd which, in turn, can cause organ complications. Cardiovascular, thrombembolic and pulmonary disease have been reported as complications in the oncological literature. Case Report: A girl aged 2 years and 4 months developed a massive intraluminal tumour of the bladder as a result of eosinophilic cystitis. Conclusions: Because of the rareness of this disease standardized therapy regimens do not exist; however, individualized treatment and close. Oncological follow-ups are highly recommended. The Aim should be to reduce the excessive production of eosinophile in order to avoid organ manifestations. Even so, the course of the disease remains unpredictable.
