Immuntherapeutische Perspektiven der Tumortherapie

Zusammenfassung

Immuntherapeutische Konzepte basieren auf der Fähigkeit des Immunsystems, autologe Tumorzellen effektiv und hochspezifisch zerstören zu können. Entscheidende Bedeutung hat die Prozessierung von Tumorantigenen durch Antigen-präsentierende Zellen sowie die suffiziente Aktivierung immunologischer Effektor-Mechanismen, an denen T-Lymphozyten, B-Lymphozyten, NK-Zellen und das Komplementsystem beteiligt sein können. Als zelluläre Therapiekonzepte werden in Studien sowohl der adoptive Transfer von ex-vivo stimulieren T-Lymphozyten als auch die spezifische Stimulation von dendritischen Zellen untersucht. Trotz überzeugender Konzepte und erstaunlicher Wirkungen bei einzelnen Patienten werden zelluläre Therapien gegenwärtig vor allem durch den hohen technischen Aufwand limitiert. Antikörpertherapien sind dagegen bereits vor allen bei Patienten mit Mamma- und Kolonkarzinomen im klinischen Einsatz etabliert. Die Wirkung beruht dabei auf der Blockade relevanter Signalwege in Tumorzellen (Trastuzumab, Bevacizumab). Neuartige Konzepte basieren auf der gezielten Bindung von Tumorzellen an radioaktiv markierte Antikörper oder der Bildung von Komplexen aus Tumorzellen und aktivierten Lymphozyten (bispezifische und trifunktionale Antikörper). Erste klinische Studien dieser so genannten „targeted therapy” zeigen viel versprechende Ergebnisse. Vor allem durch die klinischen Erfolge der Antikörper-Therapien etabliert sich Immuntherapie innerhalb multi-modaler Konzepte als neue Option der individualisierten Tumortherapie.

Abstract

Immunotherapy against cancer is based on the general ability of the immune system to specifically attack autologous tumor cells. Key factors are the suitable processing of tumor antigens by antigen presenting cells as well as the sufficient activation of immunological effector mechanisms, which are mediated by T-lymphocytes, B-lymphocytes, NK-cells and the complement system. Among cellular concepts, adoptive cellular transfer of ex vivo stimulated T-lymphocytes or specific stimulation of dendritic cells show striking efficacy in single patients, but are limited by technical factors. Antibody therapy is generally established in clinical treatment pathways, i. e. in colon and breast cancer (Trastuzumab, Bevacizumab). Novel concepts are based on “targeted therapies” by connecting radio-labelled antibodies to tumor cells or by creating a complex of tumor cells and activated T-lymphocytes, which show promising results in several studies. Regarding the clinical benefits of antibody therapies within multimodal concepts, immunotherapy will be established as a option for modern individualized tumor therapy.
