Die Bestimmbarkeit des Hüftrotationszentrums anhand palpabler Landmarken des Beckens

Zusammenfassung
Studienziel: Die Lage des physiologischen Hüftrotationszentrums in Relation zu palpablen Landmarken am Becken soll angegeben werden. Methode: Bei 50 Patienten, 25 Männern, 25 Frauen, wurden anhand von CT-Aufnahmen des Abdomens retrospektiv die Koordinaten von anatomischen Landmarken des Beckens bestimmt, wobei die Auswertung der CT-Bilder immer beidseits erfolgte. Mit Hilfe der Datensätze wurde die Lage der Hüftrotationszentren in Relation zu den Landmarken bestimmt. Ergebnisse: Das Hüftrotationszentrum liegt 12 % (± 2,9) medial, 33 % (± 3,9) distal und 19 % (± 2,7) posterior der Distanz zwischen der Spina iliaca anterior superior rechts und links von der Spina iliaca anterior superior der jeweiligen Seite entfernt. Schlussfolgerung: Mithilfe einer Computertomographie oder einer intraoperativen 3D-Fluoroskopie erscheint es möglich die Lage des physiologischen Hüftrotationszentrums abzuschätzen und Algorithmen für Navigationssysteme zu entwickeln, um intraoperativ dem Operateur zu ermöglichen das physiologische Hüftrotationszentrum zu rekonstruieren.
Abstract
Aim: The object of this study was to develop a new method to predict the location of the centre of rotation of the hip joint reasonably accurately. Method: We collected the coordinates of palpable bony landmarks in 50 patients, 25 males and 25 females, using CT scans to predict the physiological location of the centre of the hip joint centre. Results: The centre of the hip was located, on average, at 12 % (± 2.9) of the inter-ASIS (anterior superior iliac spine) distance medial, 33 % (± 3.9) distal and 19 % (± 2.7) posterior to the anterior superior iliac spine (ASIS). Conclusion: Knowledge of the physiological centre of rotation of the hip may be useful in navigation of the cup in total hip arthroplasty.
