Unusual Cause of Bilateral Optic Disc Swelling: POEMS Syndrome

Hintergrund: POEMS Syndrom (Polyneuropathy, Organomegaly, Endocrinopathy, Monoclonal protein, Skin changes) ist eine seltene Erkrankung, bei der eine Funktionsstörung der Plasmazellen im Vordergrund steht. Eine Papillenschwellung kann vorkommen, ein Makulödem wurde bisher nur einmal beschrieben [4]. Patienten und Methoden: Drei Patienten mit einem POEMS Syndrom werden beschrieben. Bei allen wurde eine Lumbalpunktion sowie eine neuroophthalmologische Untersuchung durchgeführt. Ergebnisse: Visuelle Symptome äußerten sich als Reduktion der Sehschärfe, Phtopsie und Verschwommensehen. Eine Papillenschwellung war bei allen Patienten vorhanden. Nur ein Patient hatte einen reduzierten Visus (0,2 am rechten und 0,7 am linken Auge) infolge eines Makulaödems, ein weiterer Patient mit vollem Visus hatte ebenfalls ein fluoreszenzangiografisch nachgewiesenes mildes Makulaödem. Bei diesen beiden Patienten wurde auch eine intrakranielle Drucksteigerung nachgewiesen (30 bzw. 39 cm H₂O). Das Makulaödem bildete sich unter Therapie mit Kortikosteroiden und Diamox zurück, die Papillenschwellung persistierte. Schlussfolgerung: Eine Papillenschwellung kommt im Rahmen des POEMS häufig vor, ein Makulaödem ist unter Umständen häufiger vorhanden als bisher angenommen. Interessanterweise wurde das Makulaödem bei den beiden Patienten mit erhöhtem intrakraniellem Druck beobachtet. Da das Makulaödem nur wenig ausgeprägt sein kann, wird es möglicherweise bei Patienten mit POEMS nicht immer diagnostiziert.

Abstract

Background: POEMS syndrome (polyneuropathy, organomegaly, endocrinopathy, monoclonal protein, skin changes) is a rare disorder associated with plasma cell disorder. Optic disc swelling can be present, and macular oedema has been reported once before [4]. Patients and Methods: Three patients with POEMS syndrome were investigated. Lumbar puncture and neuro-ophthalmic examination were performed on all patients. Results: Visual symptoms included decreased vision, photopsia and blurred vision. Optic disc swelling was present in all three patients. Visual acuity was decreased in only one patient (0.2 in right eye, 0.7 in left eye) due to macular oedema, and another patient with normal visual acuity showed also a mild macular oedema on fluorescein angiography. In these two patients, elevated intracranial pressure was found (30 and 39 cm H₂O, respectively). The macular oedema disappeared under steroids and acetazolamide therapy with visual improvement, but optic disc swelling remained. Conclusion: Optic disc swelling is frequent in POEMS and macular oedema may be more frequently present than previously thought. Interestingly macular oedema was detected in the two patients with increased intracranial pressure.
