Zusammenfassung
Hintergrund: Das Gorlin-Goltz-Syndrom (Basalzellnävussyndrom) ist eine seltene, autosomal dominant vererbte Erkrankung, bei der u. a. Lidbasaliome auftreten können. Berichtet wird eine außergewöhnlich schwere Manifestation bei einem Kind und der erstmalige Einsatz der Photodynamischen Therapie (PDT) bei dieser Konstellation. Patient: Bei einem 13-jährigen Patienten mit ausgeprägtem Gorlin-Goltz-Syndrom bestanden zahlreiche, z. T. konfluierende Basaliome an beiden Lidern, Hals und Rumpf. Die Basaliome des Lidbereiches waren chirurgisch nicht vollständig entfernbar. Es erfolgte beidseits eine topische PDT periorbital. Ergebnisse: Durch die PDT konnten zahlreiche oberflächliche Basaliome effektiv mit gutem kosmetischem Ergebnis entfernt werden. Schlussfolgerung: Die PDT kann eine sinnvolle Therapiealternative für Lidbasaliome darstellen, wenn diese chirurgisch nicht kurativ therapierbar sind. Verlaufskontrollen des behandelten Hautareals sind hierbei erforderlich.
Abstract
Background: Gorlin Goltz syndrome is a rare, autosomal dominant inherited disease that is characterised by multiple basal cell carcinomas (BCC) including the periorbital region and eye lids. We report a severe infantile manifestation with lid involvement treated by photodynamic therapy (PDT). Patient: A 13-year-old boy with Gorlin Goltz syndrome presented with multiple confluent BCC on both eye lids and the skin of neck and trunk. Multiple bilateral periorbital confluent and surgically not removable BCC were treated by topical PDT. Results: Numerous superficial BCC were successfully treated by photodynamic therapy with remarkable cosmetic results. Conclusion: In cases of numerous confluent and surgically not removable BCC, PDT represents an effective therapy. Frequent monitoring is necessary to maintain the clinical outcome.
