Zusammenfassung

Hintergrund: Das Krankheitsbild der retropharyngealen Tendinitis gehört zu den dystrophen Sehnenverkalkungen. Aufgrund der Beschwerdesymptomatik und einer in der MRT des Halses auffallenden Signalverstärkung prävertebral kann sie aber als Retropharyngealabszess fehldiagnostiziert werden. Methode: Wir präsentieren zwei Patientinnen, die uns mit einer solchen Fehldiagnose eines „retropharyngealen Abszesses” vorgestellt wurden. Schlussfolgerung: Durch eine sorgfältige Anamnese und Diagnostik sowie vor allem durch Kenntnis des Krankheitsbildes der retropharyngealen Tendinitis können Fehlinterpretationen der eigentlich typischen Beschwerden vermieden werden. Folglich bewahrt eine korrekte Diagnose die Patienten vor unnötigen Maßnahmen und deren möglichen Komplikationen, wie z. B. einer Lumbalpunktion, einer Operation oder unnötigen Antibiotikagaben.

Abstract

Because of their similarity, clinical symptoms of a calcific retropharyngeal tendinitis can be misdiagnosed for a retropharyngeal abscess. We present two patients who were admitted for retropharyngeal abscess, however, careful examination revealed amorphous calcification anterior to C1-C2 in addition to prevertebral soft tissue swelling. Knowledge of the characteristic clinical symptoms and the pathognomonic imaging features is crucial to correctly diagnose the disease and to prevent unnecessary tests and treatment.
