Brustaugmentation - Methoden und Bildgebung

Zusammenfassung

Silikonprothesen werden zur Augmentation aus primär kosmetischen Gründen oder zum Wiederaufbau der Brust nach einer Operation implantiert. Dieser Beitrag gibt einen aktuellen Überblick über die Rolle von Mammographie, Sonographie und Magnetresonanztomographie zur Defektbeurteilung, zur Tumorfrüherkennung und zur Nachsorge nach Prothesenimplantation. Auf geeignete Einstelltechniken für die mammographische Untersuchung von Implantatträgerinnen wird besonders eingegangen. Weiterhin werden autologe Rekonstruktionstechniken und Protokolle der sekundären und tertiären Früherkennung aufgezeigt.

Abstract

Silicone prosthesis can be implanted for cosmetic reasons or after volume reducing surgery of the breast. This paper summarizes state-of-the-art application of mammography, ultrasound and MRI for defect diagnosis, tumour screening and post-operative follow-up after implantation. In addition, tumour screening and post-surgical follow-up after autologous reconstruction are discussed.

Implantatdefekte

• Bei der Diagnostik von Implantatdefekten weist die MR-Mammographie unter den etablierten bildgebenden Untersuchungsmodalitäten die höchste Sensitivität und Spezifität auf (MR-Mammographie > Ultraschall > Mammographie), insbesondere zum Nachweis der am häufigsten auftretenden intrakapsulären Ruptur.

• Bei der Befundung von Prothesendefekten ist die Implantationsdauer und Operationsindikation zu berücksichtigen. Tastbefunde oder mammographische Auffälligkeiten bei Implantatträgerinnen sollten als Methode der ersten Wahl sonographisch korreliert werden.

• Bei klinischer Symptomatik (Formveränderung, Konsistenzveränderung, Volumenminderung, Schmerzen), längeren Implantationszeiten (> 10 Jahre) und positiver Sonographie ist eine MRT nicht zwingend erforderlich. Sind dagegen in der Sonographie fragliche Hinweise auf einen Defekt vorhanden, sollte eine MR-Mammographie folgen, um Rupturen nachzuweisen oder auszuschließen.

Tumornachsorge/Früherkennung

• Im Rahmen der Tumornachsorge und in der Früherkennung ist die MR-Mammographie nach nicht konklusiver Mammographie und Sonographie als Verfahren mit dem höchsten negativen prädiktiven Wert bezüglich einer Malignommanifestation indiziert und zur Unterscheidung Narbe versus Rezidiv ein etabliertes Verfahren.

• Die Früherkennungs- und Nachsorgeprotokolle nach Prothesenimplantation unterscheiden sich dabei nicht von Nichtimplantatträgerinnen, wobei insbesondere die mammographische Aussagekraft eingeschränkt ist.

• Ausschließlich in einer definierten Hochrisikosituation wie beim sog. genetischen Mammakarzinom wird die MR-Mammographie in Kombination mit der Sonographie und Mammographie als primäre Screeningmodalität vorgesehen.

• Nach autologen Aufbauplastiken weist die Mammographie eine hohe Sensitivität auf, die Axilla kann sonographisch beurteilt werden. Die MR-Mammographie spielt dort nur eine ungeordnete Rolle.
