Zusammenfassung
Anliegen: Die Prävalenz von Angststörungen unter Alkoholabhängigen ist hoch, doch liegen nur wenig Befunde zur Psychopathologie dieser Patientengruppe vor. In einer Vergleichsstudie sollen deshalb Charakteristika dieser komorbiden Patienten identifiziert werden, die therapeutische Relevanz haben könnten.
Methode: Verglichen wurden soziodemographische und klinische Merkmale zweier Patientengruppen (jeweils n=46) mit und ohne Angststörungen in stationärer Alkoholentzugsbehandlung. Die Symptombelastungen wurden mittels bewährter Selbstbeurteilungsskalen erfasst.
Ergebnisse: Die Befunde zur Schwere der Alkoholabhängigkeit sind uneinheitlich. Deutlicher sind Unterschiede hinsichtlich der Funktionalität des Alkoholkonsums. Patienten mit Angststörungen knüpfen häufiger positive Erwartungen an die Wirkungen des Alkohols, setzen ihn gezielter zur Affektregulierung ein und weisen einen häufigeren Missbrauch von weiteren Substanzen auf. Sie leiden häufiger unter depressiven Störungen und zeigen generell stärkere Symptome psychosozialer Beeinträchtigungen. Sie ließen sich anhand weniger Skalen auf Basis von Diskriminanzfunktionswerten hinreichend gut differenzieren.
Schlussfolgerungen: Die Entwicklung spezifischer Therapieangebote zur integrativen Behandlung von Sucht und Angst sowie ökonomischer Screening-Instrumente zur frühen Identifikation sucht-komorbider Angststörungen erscheint dringend geboten.
Abstract
Objective: The prevalence of anxiety disorders among alcoholics is high, but there are only few findings about the psychopathology of this group of patients. Therefore, to describe these comorbid patients in a comparative study may be of clinical importance.
Methods: Sociodemographic and clinical characteristics of two groups of patients (each n=46) with and without anxiety disorders taking part in an inpatient alcohol detoxification treatment were compared. Psychopathological symptoms were measured by well known self-rating scales.
Results: The findings about the severity of alcohol dependence are inconsistent. Differences with regard to a functional use of alcohol are more evident. Patients with anxiety disorders create themselves positive expectations about alcohol's effects more frequently, they use alcohol, in a more specific way, as a method of affect regulation and misuse other substances more frequently. They suffer from depressive disorders more often, and, in general, were found to have more severe symptoms of psychosocial impairment. They could be identified adequately with a few scales based on discriminant function coefficients.
Conclusions: To develop specific therapy programs for an integrative treatment of addiction and anxiety as well as economic screening-instruments for an early identification of comorbid anxiety disorders is highly desirable.
