Postreanimationstherapie: Therapiekonzepte und Prognosestellung

Zusammenfassung

Nach Herz-Kreislauf-Stillstand beträgt die Letalität in der Postreanimationsphase bis zu 70 %. Einer optimalen intensivmedizinischen Betreuung dieser Patienten kommt daher große Bedeutung zu. Dieser Beitrag fasst die aktuellen Erkenntnisse und Empfehlungen zur Postreanimationstherapie zusammen. Ein besonderer Schwerpunkt liegt hierbei auf der therapeutischen milden Hypothermie. Diese stellt aktuell das einzige Therapiekonzept dar, für das durch randomisierte klinische Studien eine Verbesserung von Überleben und neurologischer Funktion nach Herz-Kreislauf-Stillstand gezeigt wurde.

Summary

After cardiac arrest, mortality in the postresuscitation period is still up to 70 %. Therefore, optimal intensive care of these patients is of great importance. This article summarizes current knowledge and recommendations for postresuscitation care. Particular focus is given to mild therapeutic hypothermia. This is currently the only therapeutic concept for which randomized clinical trials have shown improved survival and neurological outcome after cardiac arrest.

• Eine optimale intensivmedizinische Betreuung in der Postreanimationsphase ist entscheidend für das Überleben von Patienten nach Herz-Kreislauf-Stillstand. Von herausragender Bedeutung hierbei sind Maßnahmen zum Schutz des durch die globale Ischämie geschädigten Gehirns.

• Höchste Evidenz in Form zweier randomisierter klinischer Studien hat dabei die Anwendung der therapeutischen milden Hypothermie.

• Bewusstlose erwachsene Patienten nach prähospitalem Herz-Kreislauf-Stillstand mit initialem Kammerflimmern sollten für 12-24 Stunden auf 32-34 °C gekühlt werden.

• Wahrscheinlich profitieren von der therapeutischen milden Hypothermie auch Patienten mit nicht defibrillierbaren Rhythmen, Patienten nach intrahospitalem Herz-Kreislauf-Stillstand sowie Kinder.

• Weitere intensivmedizinische Therapiekonzepte nach Herz-Kreislauf-Stillstand umfassen Normoglykämie, Normotonie, Normokapnie, koronare Revaskularisierung und antikonvulsive Therapie.

• Ein Abschätzen der Prognose des Patienten gerade in der Frühphase nach Herz-Kreislauf-Stillstand bleibt schwierig. Am ehesten sind hierzu die klinische neurologische Untersuchung sowie die Ableitung von somatosensibel evozierten Potenzialen geeignet.
