Zusammenfassung
Hintergrund und Ziel: Die Versorgung von Hüftfrakturen stellt aufgrund steigender Inzidenz, hoher Letalität und Pflegebedürftigkeit eine erhebliche Herausforderung für das deutsche Gesundheitswesen dar. Ziel dieser Studie war es, die aktuellen und zukünftig zu erwartenden jährlichen Gesamtkosten von Hüftfrakturen in Deutschland zu ermitteln.
Methodik: Gesamtkosten von Hüftfrakturen wurden aus Perspektive der Gesellschaft über Multiplikation der Lebenszeitkosten nach Alter mit der entsprechenden Inzidenz berechnet (Inzidenz-basierte Krankheitskostenstudie). Zur Berechnung der Lebenszeitkosten wurde ein Markov-Entscheidungsmodell entwickelt. Sekundärdaten wurden verwendet.
Ergebnisse: Die Analyse zeigt, dass Hüftfrakturen jährlich direkte Kosten von € 2,77 Milliarden aus gesellschaftlicher Sicht verursachen. Aufgrund der zunehmenden Alterung der Gesellschaft ist bis 2030 mit einem Anstieg auf € 3,85 Milliarden zu rechnen.
Schlussfolgerung: Die Ergebnisse legen nahe, effektive Präventionsmaßnahmen zur Vermeidung von Hüftfrakturen zu identifizieren und auf ihre Wirtschaftlichkeit hin zu überprüfen.
Abstract
Background and Objective: Treatment of hip fractures is a major challenge for the German health-care system due to the increasing incidence, high mortality rate, and need for long-term care. The purpose of this study was to determine the present and future economic burden of hip fractures in Germany.
Methods: Annual costs of hip fractures were determined from a societal perspective, by multiplying individual lifetime costs at different ages by the incidence of hip fractures (incidence-based cost-of-illness study). To calculate individual lifetime costs, a Markov decision model was developed. Secondary data were used.
Results: Total annual costs related to hip fractures were € 2.77 billion. Due to population aging, costs of hip fractures may increase to € 3.85 billion in 2030.
Conclusion: The result implies the need to identify effective prevention strategies for hip fractures and evaluate their cost-effectiveness.
