Gesundheitsökonomische Analyse der Früherkennung

Zusammenfassung

Der vorliegende Beitrag untersucht die gesundheitsökonomischen Implikationen der Früherkennung. Zunächst werden Anforderungen formuliert, die von Früherkennungsprogrammen erfüllt werden sollten, und es werden die methodischen Standards der gesundheitsökonomischen Evaluation dargestellt. Anschließend wird am Beispiel des Neugeborenen-Hörscreenings untersucht, wie den Anforderungen an Früherkennungsprogramme sowie den Standards der gesundheitsökonomischen Evaluation in der empirischen Umsetzung genügt wird. Einige Defizite sind dabei erkennbar. So werden keine randomisierten und zumeist auch keine kontrollierten Studien durchgeführt. Eine Auswirkung davon ist, dass nur durchschnittliche, aber keine inkrementellen Kosten-Effektivitäts-Relationen ausgewiesen werden können. Zudem ist belastbares Wissen aus Studien über langfristige Ergebnisse von Hörscreening, Früherkennung und Frühversorgung kaum vorhanden. Die Schlussfolgerung ist, dass ein erheblicher Bedarf an Studien zu den langfristigen Ergebnissen des Hörscreenings besteht. Ebenso besteht ein Bedarf an Vergleichsstudien von Screening mit Nichtscreening, um die inkrementelle Kosteneffektivität abbilden zu können.
