In most cases, inhalation therapy with saline solutions is an essential part in the treatment of obstructive respiratory tract disease. The best composition and concentration of the solution to be used is still controversial.
The present double-blind study investigated into effect and tolerance of inhalation therapy with native and diluted sea water (salt concentrations 3.5 and 1.75 %, respectively) in comparison with physiological saline and a control group without inhalation therapy. 120 patients divided into three groups according to the severity of their obstructive respiratory tract disease took part during a stay of 4 to 6 weeks in a sanatorium on a North Sea island. Airway resistance was measured at the beginning and the end of inhalation therapy as well as before, immediately and 10 minutes after the first and the last single inhalations.
After at least 15 inhalations, no therapeutic effect could be proved. Pulmonary function measurement in the course of the first and the last single inhalations showed a pattern typical of hyperosmolar solutions. A slight, statistically significant, and a few cases more intense, increase in respiratory tract resistance was completely reversible 10 minutes after inhalation.
Inhalation therapy with hyperosmolar saline is well tolerated even by patients with severe obstructive respiratory tract disease. An immediate reaction should be regarded as a desired therapeutic effect representing a natural physical hyposensitisation with bronchial hyperreactivity already existing.
Zusammenfassung
Eine Inhalationstherapie mit Salzlösungen ist in den meisten Fällen fester Bestandteil des Behandlungsprogrammes bei obstruktiver Atemwegserkrankung. Kontrovers diskutiert werden die optimale Zusammensetzung und Konzentration der zu verwendenden Lösung.
Die vorliegende, doppelblind im Rahmen eines Meeresklimaheilverfahrens durchgeführte Studie untersuchte an 120 Patienten, die nach Schweregrad der obstruktiven Atemwegserkrankung in drei Gruppen eingeteilt wurden, die Wirksamkeit und Verträglichkeit einer Inhalationstherapie mit nativem und verdünntem Meerwasser (3,5 % bzw. 1,75 % Salzgehalt) im Vergleich zu physiologischer Kochsalzlösung und einer offenen Kontrollgruppe ohne Inhalationsbehandlung.
Als Hauptzielparameter wurde bodyplethysmographisch die Resistance zu Beginn und Ende des Heilverfahrens sowie mittels Unterbrechermethode vor, direkt und 10 Minuten nach erster und letzter Einzelinhalation gemessen.
Eine therapeutische Gesamtwirkung der täglich, insgesamt mindestens 15mal durchgeführten Inhalationen ließ sich nicht sichern. Die Lungenfunktionsmessungen im Inhalationsverlauf ergaben ein typisches Reaktionsmuster bei den hyperosmolaren Lösungen. Ein geringer, statistisch signifikanter und in wenigen Einzelfällen auch stärker ausgeprägter Anstieg der Resistance war 10 Minuten nach Inhalation vollständig reversibel.
Inhalationen mit hyperosmolaren Salzlösungen sind auch für Patienten mit schwergradiger obstruktiver Atemwegserkrankung generell gut verträglich. Eine Immediatreaktion auf den Inhalationsreiz ist im Sinne einer natürlichen Hyposensibilisierung bei vorbestehender bronchialer Hyperreagibilität als therapeutisch erwünscht anzusehen.
