Zusammenfassung
Im Folgenden wird ein 9 Jahre alter Patient vorgestellt, welcher bei einem Schwimmbadunfall auf die Oberkieferfront gestürzt war. Die Zähne 11 und 21 erlitten dabei eine ausgeprägte intrusive Dislokation nach vestibulär sowie eine Schmelz-Dentinfraktur ohne Pulpabeteiligung. Fünf Monate nach dem Unfall stellte der Patient sich erstmals in der Poliklinik für Kieferorthopädie der Justus-Liebig-Universität vor, da eine Spontaneruption der traumatisierten Zähne ausgeblieben war. Im Rahmen der kieferorthopädischen Behandlung wurden 11 und 21 vorsichtig mobilisiert und extrudiert, zunächst mittels vertikaler Gummizüge an einer Oberkieferplatte. Die Feineinstellung erfolgte durch Teilbebänderung im Oberkiefer mit abschließender konservierender Kronenrekonstruktion. Die Einordnung der traumatisierten Zähne verlief komplikationslos unter Erhalt der Vitalität.
Abstract
Early orthodontic treatment of a 9-year-old patient is described. Due to a swimming pool accident teeth 11 and 21 had been severely intruded and displaced labially and had suffered dentine-enamel fractures without pulp involvement. Five months after the accident the patient was referred to the Orthodontic Department of the University of Giessen, since no spontaneous eruption of the intruded teeth had occurred since then. During orthodontic treatment teeth 11 and 21 were carefully mobilized and extruded, first, with vertical elastics pulled to an upper removable plate. The final alignment was performed with partial multibracket appliances followed by composite crown reconstructions. Both incisors could be aligned without complications and under maintenance of vitality.
