Zusammenfassung
Anamnese und körperliche Untersuchung sind Schlüsselfunktionen für das Erfassen der
Probleme, mit denen der Patient zum Arzt kommt. Die korrekte Erfassung und Fixierung
der Ergebnisse sind die Voraussetzung für eine gezielte und kostensparende Diagnostik
und Therapie. Die Weitergabe der gesammelten Ergebnisse kann mündlich, schriftlich
analog oder digital stattfinden. Wichtig ist die lückenlose Dokumentation nach einem
festgesetzten Schema. Neue digitale Programme helfen dabei, dieses Ziel zu erreichen,
binden jedoch den Arzt bei der Dokumentation und Entfernen ihn vom Patienten. Nur
die gemeinsame Informationserfassung von Schwestern und Ärzten wie zum Beispiel bei
der gemeinsamen Visite führt zu einer zielgerichteten und kostengünstigen Diagnostik
und Therapie des Patienten. Wichtig ist es, das die verschiedenen Berufsgruppen die
gesammelten Informationen nutzen und den Patienten als gemeinsamen Patienten wahrnehmen.
Abstract
Anamnesis and clinical examination are the key functions of medical doctors to reveal
the health problems of their patients. The correct assessment and handoff of these
informations are the preconditions for a specific and cost saving diagnostic and therapy.
The handoff can be made orally, in written form analogue or digital. The examination
and documentation should be conducted in the order specified for every patient to
avoid mistakes. New digital programs help to reach this aim but absorb the time of
the medical doctor and depart him from the patients. Nurses and medical doctors should
perform the rounds together for a mutually acquisition of information. This conduces
towards a single-minded and cost-effectively diagnostic and therapy.
