Am Beispiel Rückenschmerzen - Psychologische und Psychobiologische Modelle der Schmerzchronifizierung

Zusammenfassung

Etwa 80 % der Bevölkerung in den westlichen Industrienationen leiden einmal in ihrem Leben unter akuten, starken Rückenschmerzen, die sich in den meisten Fällen innerhalb weniger Tage mit einfachen Maßnahmen wieder zurückbilden. Jedoch entwickeln bis zu 35 % der Betroffenen mit der Zeit chronische Beschwerden mit immer häufiger wiederkehrenden oder anhaltenden Schmerzen. Im vorliegenden Beitrag wird ein Überblick über den Stand der psychologischen bzw. psychobiologischen Forschung zur Chronifizierung von Rückenschmerzen gegeben. Unter den psychologischen Einflussfaktoren gehören die depressive Stimmung, chronisch anhaltende Belastungen im privaten oder beruflichen Alltag sowie ungünstige Formen der Schmerzverarbeitung zu den zentralen Mediatoren der Chronifizierung. Des weiteren werden mögliche psychobiologische Verknüpfungen und präventive Interventionen diskutiert.

Summary

The life time prevalence of acute severe back pain is about 80 % in the industrial nations. Nearly 35 % of these people will develop chronification with recurrent or persistent pain. This review gives an overview about the role of psychological and biopsychological factors in this process. Among the psychological factors, depressive mood, chronic daily stress in private or job life and maladaptive modes of pain processing and coping behavior are relevant mediating factors of chronicity. Finally possible psychobiological links and preventive interventions are discussed.
