Prächirurgische Diagnostik und Epilepsiechirurgie im Kindesalter - Spezielle Aspekte

Die Prinzipien der prächirurgischen Diagnostik sind bei Erwachsenen und Kindern grundsätzlich ähnlich. Der Anteil extratemporaler Epilepsien ist im Kindesalter höher, darüber hinaus ist die Durchführung des komplizierten diagnostischen Prozesses bei Kindern schwieriger. Sehr schwere, häufig sich schon im Säuglingsalter manifestierende Epilepsien auf dem Boden von Fehlbildungen, Rasmussen- Enzephalitis und Folgezuständen verschiedenartiger Schädigungen, verlangen ein rasches Handeln, da die Entwicklung dieser Kindern nicht nur auf Grund der vorbestehenden zerebralen Läsionen, sondern auch wegen der hohen Anfallsdichte und interiktualen EEG-Veränderungen außerordentlich gefährdet ist.

The principles of the presurgical diagnostic and epilepsy surgery are similar in adults and children. The rate of extratemporal epilepsies is higher in infancy and the carrying out of the complex diagnostic process is more difficult in young children. Severe, refractory epilepsies in infancy and childhood require quick decisions, because the development of these children not only depends on the underlying cerebral lesion, but also of high seizure frequency and the extent of interictual spike activity.
