Abstract
This study aims at clarifying why the apophysis of the greater trochanter very rarely separates, in contrast to other apophyses of the hip region.
The inclination and area measurement of the greater trochanteric growth plate and the mode of insertion of the muscles on the apophysis were analyzed on the basis of 16 anatomic femoral specimens from newborn to children of 14 years of age. The physiological muscle cross section Q of the muscles inserting at the greater trochanter was determined on 6 specimens. In a cross-sectional radiological study, carried out on 1350 hip joints of healthy children, the inclination of the greater trochanter growth plate was measured.
The anatomical and radiological findings show that the nearly plane-shaped greater trochanter growth plate remains inclined at a 50 degree angle to the horizontal body line and is loaded from a diagonally craniolateral direction throughout the total growth period. The lateral surface of the apophysis is covered by a fibrous connection which joins the insertion areas of the gluteus medius, minimus and vastus lateralis muscles. The vastus lateralis muscle is intimately bound to the vastus intermedius muscle by fibrous tissue. According to the results of the physiological muscle cross sections these four muscle groups can form a counteracting muscle sling, which transforms the traction forces at the surface of the greater trochanter into pressure forces in line with a tension band effect.
On the basis of readings, measurements and computations, one can presume that the apophysis of the greater trochanter can only be separated toward a ventral or dorsal direction by means of a severe trauma externally affected, but not avulsed as a result of sudden contractions of the muscles.
Zusammenfassung
In der vorliegenden Untersuchung sollte geklärt werden, warum die Apophyse des Trochanter major im Gegensatz zu anderen Apophysen des Hüftbereichs extrem selten disloziert. Es wurden anhand eines anatomischen Untersuchungsgutes 16 kindlicher und juveniler Femora Ausrichtung und Flächenmaß der Apophysenplatte sowie der Anheftungsmodus der an der Apophyse ansetzenden Muskulatur analysiert. An 6 Präparaten wurde der physiologische Muskelquerschnitt Q der Trochantermuskulatur bestimmt. An 1350 Beckenübersichten gesunder Kinder wurde die Lage der Apophysenplatte vermessen.
Die anatomischen und radiologischen Ergebnisse zeigen, daß die annähernd plan ausgebildete Apophysenplatte während des gesamten Wachstums im 50 Gradwinkel zur Körperhorizontalen eingestellt bleibt und von schräg lateral-kranial her druckbeansprucht wird. Die Außenseite der Apophyse ist durch eine Faszienplatte bedeckt, die die Insertionsareale der Mm. glutaei medii, minimi et vastus lateralis vereinigt. Der M. vastus lateralis ist in seinem weiteren Verlauf faszial eng mit dem M. vastus intermedius gekoppelt. Diese vier Muskelgruppen können entsprechend den Ergebnissen der physiologischen Muskelquerschnittsbestimmung eine gegenziehende Muskelschlinge bilden, die Zug an der Spitze des Trochanter major nach dem Zuggurtungsprinzip in Druck umwandelt.
Aufgrund des Literaturstudiums und aufgrund experimenteller Überlegung ist anzunehmen, daß die Apophyse des Trochanter major nur durch ein erhebliches von außen einwirkendes Trauma nach ventral oder dorsal dislozieren, nicht jedoch durch plötzliche Muskelanspannung abreißen kann.
