Tinea pedum und verspätet erkannte Zehengangrän bei peripherer arterieller Verschlusskrankheit

Zusammenfassung

Ein Patient wurde unter der Diagnose einer Tinea pedum durch Trichophyton rubrum von einem Dermatologen fachgerecht topisch antimykotisch behandelt. Bei Therapieresistenz und Befundverschlechterung erfolgte eine Umstellung der Lokaltherapie, jedoch keine weitere Diagnostik bez. ätiologischer und Risiko-Faktoren. Eine Vorstellung bei einer chirurgischen Praxis führte zur Diagnose einer umschriebenen Nekrose der 3. Zehe rechts bei bekannter peripherer arterieller Verschlusskrankheit und Immunsuppression mit nachfolgender Zehenamputation und kompliziertem Wundheilungsverlauf. Nach Facharztstandard lag ein Befunderhebungsfehler vor, der rechtlich zu einer Beweislastumkehr führt, sodass die Gesundheitsschädigung in Form des Voranschreitens einer Weichteil- und Knocheninfektion bei umschriebener Nekrose der 3. Zehe rechts mit Amputation dem Arzt zuzurechnen war.

Therapieresistente Fußmykosen und infizierte Fußläsionen erfordern unverzüglich intensive Behandlungsmaßnahmen und eine Fahndung nach Risikofaktoren wie eine gefäßdiagnostische Kontrolle.

Abstract

A patient was diagnosed with tinea pedum due to Trichophyton rubrum and treated by a dermatologist with a topical antifungal. When the condition proved to be therapy-resistant and the symptoms worsened, the topical therapy was changed, but no further diagnostic examinations were undertaken with regard to aetiological and risk factors. When the patient presented to a surgeon, a diagnosis of circumscribed necrosis of the 3rd toe with known peripheral arterial occlusive disease and immunosuppression was made, followed by a toe amputation and complicated wound healing. According to the medical specialist standard, there was a diagnostic error which legally leads to a reversal of the burden of proof, so that the damage to the patient’s health in the form of progression of the soft tissue and bone infection with circumscribed necrosis of the 3rd toe and amputation was attributed to the dermatologist.

Therapy-resistant foot mycoses and infected foot lesions require immediate intensive treatment measures and a search for risk factors such as vascular diseases.
