Zusammenfassung
Um den Handlungsspielraum bei der Versorgung von traumainduzierten Akutschmerzen für das ärztliche und nichtärztliche Rettungsdienstpersonal zu erweitern, etablierten wir einen Algorithmus zur Versorgung von traumainduzierten Akutschmerzen. Das seit 2018 in Deutschland zugelassene Methoxyfluran (Penthrox) verwenden wir als Analgetikum für mittelstarke bis starke Schmerzen bei Erwachsenen ohne Bewusstseinseinschränkung. Methoxyfluran ist selbst titrierend, schnell anflutend, inhalativ, nebenwirkungsarm. Neben einem Literaturüberblick über die jahrzehntelange Erfahrung mit dessen prähospitalem Einsatz im Ausland werden 2 Fallbeispiele aus unserer klinischen Praxis vorgestellt. Methoxyfluran ist ein Nichtopioid und kann von geschultem nichtärztlichen Rettungsdienstpersonal angewendet werden. Die Applikationsform ermöglicht eine Schmerztherapie ohne Zeitverlust, bis ein intravenöser Zugang für eine additive Therapie etabliert wird.
Abstract
To optimize early pain therapy in trauma and severe trauma patients in a prehospital setting, we established an algorithm for improved pain management by paramedics and casualty doctors. Methoxyflurane (Penthrox), approved since 2018 in Germany, is from now on taken into consideration for pain therapy from moderate to severe pain in adults without loss or transient loss of consciousness. Methoxyflurane is self titrating, quickly washing in, inhaled, and reveals only minor side effects. Besides an overview of the literature to the past decades of usage in a prehospital setting overseas, we present two brief case reports of our experience in the usage of methoxyflurane. Methoxyflurane is a non-opioid analgesic drug and may safely be administered by trained emergency service personnel. Due to its characteristics, methoxyflurane may be applied rapidly even before an i. v.-line is established for further treatment of trauma and severe trauma patients without loss of
consciousness.
