Einführung

Veränderungen der Hämodynamik in der Leber beruhen am häufigsten auf einer Leberzirrhose, dem Budd-Chiari-Syndrom oder kongenitalen Gefäßanomalien [1], [2], [3], [4]. Arterioportale Fisteln (APF) sind abnormale Gefäßverbindungen zwischen Arterien und dem Pfortadersystem und treten typischerweise in Zusammenhang mit einer Leberzirrhose oder dem hepatozellulären Karzinom auf, welches häufig in die Pfortader hinein wächst [5]. Die APF stellt die häufigste vaskuläre Komplikation einer perkutanen Leberbiopsie dar. Tatsächlich beträgt die Inzidenz von arterioportalen Fisteln nach Leberbiopsie bis zu 5,2% [6]; die Mehrzahl bleibt aber klinisch asymptomatisch und erfordert keinerlei Behandlung. APF kann intra- oder extrahepatisch auftreten und erworben oder angeboren sein [5], [6], [7], [8], [9]. Eine APF mit hohem Fluss weist einen variablen Anteil von retrogradem Fluss durch die Pfortader auf [7]. Obwohl dies asymptomatisch sein kann, besteht eine gewöhnlich präsinusoidal lokalisierte portale Hypertension, die letztendlich zu Oesophagusvarizen, Blutungen und Aszites führt, wenn ein großes Shuntvolumen durch die Fistel fließt [12]. Wir berichten über den Fall einer APF mit hohem Fluss, die sich 25 Jahre nach einem operativ behandelten Lebertrauma manifestierte.
