Abstract
Cranial computerized tomography was performed in 145 patients with petit mal epilepsies. Tomographic results were compared to clinical data. 59 patients (41%) had normal findings, in 39 children (27%) we found slight pathologic changes. In 33 (22%) medium degree changes, and in 14 children (10%) severe pathologic changes were seen.
West syndrome : (N = 19). 18 patients with pathologic findings. The combination of cortical plus subcortical atrophies (N = 11) was observed most frequently. Atrophies were of mostly severe or medium degree.
Lennox syndrome : (N = 31). 28 patients (90%) had pathologic results, subcortical atrophies (N = 19) prevailed.
Centrencephalic myoclonic-astatic petit mal : (N = 43). 26 normal and 17 (40%) pathologic findings at CTAT. Pathologic findings were mostly grouped as slight (N = 13), with subcortical atrophies prevailing again. However, there was a statistically significant difference to Lennox syndrome as far as frequency and severity of pathologic changes are concerned.
Absence epilepsy : (N = 49). 29 normal results and 20 (41%) pathologic findings. Slight atrophies were prevailing (N = 14).
More favourable findings could be secured statistically in the following subgroups:
In primary generalized petit mal epilepsies as compared to secondary generalized types. This applies to frequency as well as severity of pathologic findings.
In centrencephalic myoclonic-astatic petit mal during infancy as compared to children with secondary generalized petit mal during this age.
In petit mal seizures without concomitant grand mal seizures if compared to the group with concomitant grand mal.
In cryptogenetic primary generalized petit mal epilepsies if compared to non cryptogenetic forms.
In patients without microcephalus if compared to those with microcephalus.
Zusammenfassung
Computertomographische Befunde des Schädels bei 145 Kindern mit Petit Ma1 Epilepsien: Propulsive Petit Mal, myoklonisch-astatisches Petit Mal, Absenceepilepsie.
145 Patienten mit Petit Ma1 Epilepsien wurden computertomographisch untersucht und die Ergebnisse mit klinischen Daten verglichen. 59 Patienten (41%) hatten unauffällige Befunde, 39 Kinder (27%) wiesen leichte, 33 (22%) mäßige und 14 (10%) wiesen schwere Veränderungen im Computertomogramm auf.
Propulsive Petit Mal: (N = 19). 18 pathologische CT-Befunde. Es überwogen deutlich die Kombinationen von kortikalen und subkortikalen Atrophien (N = ll), meist mittleren bis schweren Grades.
Lennox Syndrom: (N = 31). 28 (90%) pathologische Befunde, deutliches Überwiegen subkortikaler Atrophien (N = 19).
Centrenzephales myoklonisch-astatisches Petit Ma1 (N = 43). 26 normale und 17 (40%) pathologische CT-Befunde. Die pathologischen Befunde waren überwiegend leicht (N = 13), subkortikale Atrophien überwogen. Die Unterschiede zum Lennox -Syndrom waren sowohl bezüglich der Häufigkeit als such der Schwere der Veränderungen statistisch signifikant.
Absenceepilepsien: (N = 49). 29 Normalbefunde und 20 (41%) pathologische Resultate. Überwiegend fanden sich leichte Atrophien (N = 14).
Statistisch signifikant günstigere Ergebnisse zeigten:
Primär generalisierte gegenüber sekundär generalisierten Petit Ma1 Epilepsien, sowohl bezüglich der Häufigkeit als auch der Schwere pathologischer Befunde.
Centrenzephales myoklonisch-astatisches Petit Ma1 im Säuglingsalter gegenüber Kindern mit sekundär generalisierten Petit Ma1 Epilepsien in diesem Lebensalter.
Petit Ma1 Anfälle ohne Grand Ma1 Anfälle gegenüber solchen, die von Grand Ma1 Anfällen begleitet waren.
Kryptogenetische, primär generalisierte Petit Ma1 Epilepsien gegenüber nicht kryptogenetischen.
Patienten ohne Mikrozephalus gegenüber solchen mit Mikrozephalus.
