Indocyanine Green Angiography Findings in Acute Retinal Necrosis Syndrome

Zusammenfassung

Hintergrund: Das akute retinale Nekrosesyndrom wird klinisch definiert durch die Anwesenheit einer peripheren nekrotisierenden Retinitis, welche mit einer schweren okklusiven Vaskulitis assoziert ist und durch einen Herpes simplex oder einen Varizella Zoster Virus verursacht wird. Bei der als ausschließlich retinal beschriebenen Pathologie wurde eine choroidale Mitbeteiligung, welche wir mit der Indozyaningrün-Angiografie demonstrieren konnten, bisher nicht ausführlich untersucht. Anamnese und Befund: Die Indozyaningrün-Angiografie wurde bei vier Patienten mit einer akuter retinalen Nekrose durchgeführt. Beobachtete angiografische Muster beinhalten: 1. ein charakteristisches dreieckiges hypoperfundiertes Gebiet; 2. hypofluoreszente lobuläre Gebiete und choroidale vaskuläre Hyperfluoreszen und 3. isolierte hypofluoreszente lobuläre Gebiete des kontralateralen Auges. Therapie und Verlauf: Die choroidalen Hypoperfusionen in der Indozyaningrün-Angiografie sind mit extensiven retinalen Ischämien vereinbar. Die Behandlung bestand aus einer antiviralen Therapie und Kortikosteroiden in Verbindung mit einer Prophylaxe mit Acetylsalicylsäure. Schlussfolgerungen: Die Indozyaningrün-Angiografie liefert wichtige Informationen einer choroidalen Beteiligung beim Akuten retinalen Nekrosesyndrom. Sie ermöglicht auch, subklinische entzündliche Zeichen des kontralateralen Auges aufzuzeigen.

Abstract

Background: Acute retinal necrosis syndrome is clinically defined by the presence of peripheral necrotizing retinitis associated with severe occlusive vasculitis caused primarily by herpes simplex virus and varicella zoster virus. Previously considered as an exclusively retinal pathology, choroidal involvement, as demonstrated by indocyanine green angiography, has not been extensively studied. History and Signs: Indocyanine green angiography was performed in 4 patients with ARN. Observed angiographic patterns included: 1. a characteristic triangular area of hypo-perfusion, 2. hypofluorescent lobular patches and areas of fuzzy choroidal vascular hyperfluorescence, and 3. isolated hypofluorescent lobular patches of the contralateral eye. Therapy and Outcome: Marked choroidal hypo-perfusion on indocyanine green angiography was associated with extensive retinal ischemia. Treatment included a combination of antiviral agents and corticosteroids complemented by prophylactic acetylsalicylate. Conclusion: Indocyanine green angiography may provide important information regarding choroidal vascular involvement in ARN. It may also permit the timely identification of sub-clinical contralateral eye involvement.
