Aktinische Keratosen – Aktuelle Entwicklungen in der Therapie

Zusammenfassung

Zur topischen Behandlung von aktinischen Keratosen (AK) sind neue Substanzen beziehungsweise Formulierungen seit kurzem verfügbar, die die bereits relativ große Auswahl an unterschiedlichen Therapieverfahren noch mal bereichern. Dies kommt der demografisch bedingten ansteigenden Prävalenz von AKs wie auch dem durch eine verbesserte Aufklärung zunehmenden und mit höheren Ansprüchen verbundenen Therapiewunsch entgegen.

Für die läsionsgerichtete Therapie steht jetzt die Wirkstoffkombination 5-Fluorouracil (0,5 %) und Salizylsäure (10 %) in Lösung als zugelassenes Arzneimittel zur Verfügung. Im Bereich der photodynamischen Therapie sind zwei neue 5-Aminolävulinsäure enthaltende Formulierungen mit nachgewiesener hoher Wirksamkeit hinzugekommen, eine davon in Pflasterform und eine als gelartige Nanoemulsion. Imiquimod wird demnächst in zwei niedrigeren Konzentrationen mit einem beschleunigten Anwendungsschema verfügbar sein. Außerdem wird die Zulassung eines nur für 2 – 3 Tage anzuwendenden Gels mit dem Wirkstoff Ingenolmebutat erwartet.

Abstract

Recently new substances and formulations have become available for the topical treatment of actinic keratoses (AK), which enrich the already relatively great choice of different treatment options. This accommodates with the demographically increasing prevalence of AKs and the rising demand of therapy due to improved education.

For the lesion-directed therapy a soluble combination of 5-fluorouracil (0.5 %) and salicylic acid (10 %) is now available as an approved drug. In the field of photodynamic therapy two new formulations containing aminolevulinic acid with proven high efficacy have been added, one of which as a medicated plaster and the other as a gel-like nanoemulsion. Imiquimod will soon be available in two lower concentrations with an accelerated application scheme. Furthermore the approval of a new gel containing ingenol mebutate is expected, which is applied for only 2 – 3 days.
