Die Sarkoidose ist eine generalisierte Erkrankung mit einer über 90 %igen Beteiligung der Lunge. Unverändert besteht die systemische Basistherapie der Erkrankung aus der Gabe eines hochdosierten Kortikosteroids. Die Heterogenität der Erkrankung und die vielen klinischen Phänotypen und der sehr unterschiedliche Organbefall erfordert ein individuell abgestimmtes Therapiekonzept. Ist die Erkrankung durch Steroide alleine nicht zu beeinflussen oder besteht eine Steroidintoleranz muss die Behandlung mit einem immunsuppressiven Medikament ergänzt werden. In dieser Übersicht werden die aktuelle Datenlage zur Sarkoidosetherapie erläutert und entsprechende Therapieempfehlungen gegeben.
