A 2-day-old male newborn, weighing 2 kg, was admitted with his left testis popping out from neck of the left hemiscrotum ([Fig. 1]). The testicular size was appropriate for age (1.5 × 1 cm). It was hanging from normal looking cord structures. However, testis and cord was covered with a yellowish slough. The right testis and penis was normal. Gubernaculum was normal in appearance. No other congenital anomaly could be noted. On exploration, after washing with saline, slough could be removed and testis could be dissected easily from adjoining structures. Dartos pouch was created and orchidopexy was performed.

Postoperative recovery was uneventful. Follow-up up to 1 year showed bilaterally symmetrical gonads of adequate size.
