Zusammenfassung
Wir berichten über eine Patientin mit langjährig bekannter Psoriasis vulgaris, die nach ca. fünf-monatiger Ustekinumab-Therapie erstmals eine atopische Dermatitis entwickelte, welche in unmittelbarem Zusammenhang mit der Applikation des neuen Medikaments (Hautveränderungen jeweils ca. 2 – 3 Wochen nach Subkutangabe) aufgetreten war. Zuvor war keine Erkrankung aus dem atopischen Formenkreis bekannt. Möglicherweise bewirkt Ustekinumab über die Hemmung der TH1-Immunantwort einen Switch zu einer überwiegend TH2-vermittelten Immunreaktivität, die bei okkulter atopischer Diathese eine atopische Dermatitis auszulösen vermag. Ähnliche Phänomene sind bei TNF-alpha-Blockade beschrieben, allerdings bisher kaum über den IL-12- und IL-23-Signaltransduktionsweg, der durch Ustekinimab gehemmt wird.
Abstract
We report on a 51-year-old woman with long lasting psoriasis vulgaris who developed a generalized atopic dermatitis five months after the beginning of an Ustekinumab-therapy. The skin lesions occurred each time 2 – 3 weeks after subcutaneous application of the drug. She had never before any atopic disease. Ustekinumab inhibits Th1 mediated immunomechanisms which may in turn induce a consecutive switch to a more Th2 dominated reactivity, thus leading to manifestation of atopic dermatitis. Similar reactions have been described in TNF α blockers, but rarely after inhibition of IL-12 and IL-23 mediated reactions by Ustekinumab.
