Zusammenfassung
Einwesentlicher Fortschrittbei derErforschung von Gefäß-anomalienbedeutetezunächst die strikteTrennung derHämangiomevon denGefäßfehlbildungen. Da es außerden häufigenrelativ banalenHämangiom-Formen(haemangiomas of infancy)eineganze Reihe seltener, in ihremVerlauf sehr unterschiedlicher Hämangiom-Formengibt, die auchsehr unterschiedlicher Behandlungsarten bedürfen, wurde zunächst beider ISSVA (International Society for the Study of VascularAnomalies), die sich mit derErforschung von Gefäßanomalienbefasst, die BezeichnungHämangiom“durch die Bezeichnung vaskulärerTumor“ersetzt. Zurweiteren Klassifizierungwurdendanninerster Linie histologischeMerkmale herangezogen.
Eine solche Einteilungist zwar sinnvollfür die Forschung, aberwenighilfreich fürden täglichenpraktischenGebrauch. Ich bevorzuge daher eine Einteilung, die sichzunächst nach klinischen Merkmalenorientiert, zumal in den meisten Fällenzur Diagnosestellungeinehistologische Untersuchungentbehrlich ist.
Summary
An importantstep in the researchonvascular anomalies wasthe strict separation of hemangiomas from vascular malformations. Because of the finding that apartfrom the “haemangiomas of infancy”which occur frequentlyand are generally relatively harmless there existsawide range of other rare haemangiomas which varyconsiderably in their clinicalcourse andrequire different kindsoftreatment, the ISSVA decided to replacethe term“haemangioma” with the term“vascular tumour”. For further distinctionsmainly histological characteristics wereused. This provedtobevaluablefor research but notveryhelpful forthe practical use in daily clinical routine. Therefore, I prefer aclassification basedonclinicalcharacteristics. Furthermore, inmost casesdiagnosis is possible without additional histology.
Résumé
La distinction stricte entreles hémangiomes et lesmalformationsvasculaires ad’abord permis un progrèsconsidérabledanslarecherchedes anomaliesvasculaires. Etant donné que l’on adécouvert qu‘il existe, àcôtédes formesd‘hémangiomes souventrelativement banales (haemangiomas of infancy), également touteune série d’hémangiomes plus rares, trèsdifférents du pointdevue de leur évolution, qui nécessitent également desformesde traitements très différentes, l’ISSVA , qui s‘occupe de la recherched‘anomaliesvasculaires, adécidé de remplacerle terme«hémangiome» par la désignation «tumeur vasculaire». Les classifications plusapprofondiesont été ensuite basées principalement sur les caractéristiques histologiques.
Une telleclassification estcertes judicieuse danslarecherche, mais pas danslapratique clinique de tous les jours.C‘est pourquoi je préfèreune classification qui repose en premier lieu sur lescaractéristiquescliniques, d‘autant plus que danslaplupart descas, un examenhistologique estindispensable pour établir le diagnostic.
