Zusammenfassung
In das Gründerjahrzehnt (1949 bis 1960) fällt die Entdeckung der Lithiumwirkung, von Neuroleptika, trizyklischen und MAO-hemmenden Antidepressiva, Methylphenidat und Benzodiazepinen; 1972 folgte noch Clozapin. Eine vergleichbare bahnbrechende Entwicklung hat es seither nicht mehr gegeben. Folgende Ursachen lassen sich für diese Diskrepanz identifizieren: In den 1950er-Jahren stimulierte das Fehlen von Therapien Forscherdrang und Experimentierfreude. Bei den schwer kranken Anstaltspatienten ließ sich die Wirksamkeit eines neuen Pharmakons schnell erkennen. Fehlende rechtliche und administrative Vorgaben ermöglichten ein ethisch problematisches Ausprobieren und Zufallsbefunde. Erst nachfolgend wurden die Risiken der Pharmakotherapie deutlich (z. B. Contergan-Skandal, massenhafte Benzodiazepinabhängigkeit). Heute hemmen umfangreiche juristische und administrative Rahmenbedingungen Einzelforscher und Institutionen mit geringem Budget. Bis zum AMNOG 2011 motivierten falsche ökonomische Anreize Unternehmen, auf Scheininnovationen zu setzen. Bahnbrechende Erfolge sind in den nächsten Jahren unwahrscheinlich; die systematische, optimierte Anwendung der verfügbaren Optionen ist das Gebot der Stunde.

In the founders’ decade (1949 to 1960) fell the discovery of the lithium effect, of antipsychotics, of tricyclic and MAO-inhibiting antidepressants, of methylphenidate and of benzodiazepines; clozapine followed in 1972. There has been no comparable breakthrough since. Numerous causes can be identified for this discrepancy: The nearly complete absence of therapeutic options in the 1950ies stimulated scientists. Effects of new drugs could be detected fast in seriously ill patients of the asylums. The absence of a juridical and administrative framework allowed ethically problematic trial and error approaches, and serendipity. Only in the following years, the risks of pharmacotherapy became clear (e.g. thalidomide scandal, epidemic benzodiazepine dependence). Today, extensive juridical and administrative requirements hold back single researchers as well as institutions suffering from budget limitations. Up to the introduction of the German AMNOG law in 2011, misleading economic incentives stimulated pharmaceutical companies to develop me-too drugs. Revolutionary progress is unlikely in the next years: what is called for is the systematic, optimized use of the options available.
