Endoscopic subserosal dissection of a large gastric GIST with risk factors
Abstract Text An 87-year-old patient with an intraluminal-predominant gastric GIST displaying risk factors on endoscopic ultrasound (growth from 19mm to 31mm in 2 years and a necrotic focus). Endoscopic dissection around the GIST was performed using Splash M Knife. Due to involvement of the muscularis propria, subserosal dissection was performed. Endoscopic suturing with Apollo Overstitch was carried out. Due to the large lesion size (5cm), sample fragmentation was necessary for oral extraction. Pathology report revealed a low-risk GIST (<5 mitoses/50 HPF, no necrotic foci).Endoscopic treatment of gastric GISTs>3cm could be considered as an alternative in individualized cases.
