Der Transpositionslappen nach Limberg in der operativen Therapie des chronischen Sinus pilonidalis

Surgery of pilonidal sinus using the Limberg flap

Summary

The operative treatment of pilonidal sinus is characterized by an increased rate of wound healing complications and late recurrences. The Limberg transposition flap is an easy method for covering the defect after excision of the pilonidal sinus. The flap smooths out the deep skin fold of the rima ani and counteracts in this manner an important pathogenetic factor of pilonidal sinus recurrence. Between 6/96 and 7/99 we treated 42 patients using this method without complications of wound healing or recurrences. Comparison of literature results of different operative treatments (open granulation, primary suture, Z-plasty) shows lessened complication and recurrence rates for the Limberg transposition flap, which is the method of choice for chronic pilonidal sinus nowadays.

Zusammenfassung

Die operative Therapie des Sinus pilonidalis ist mit einer erhöhten Rate an Wundheilungsstörungen und Rezidiven belastet. Der Transpositionslappen nach Limberg ist eine leicht durchzuführende Methode zur Deckung des Weichteildefektes nach Exzision des Sinus, die zudem eine tiefe Analfalte glättet und somit dem wesentlichen Faktor im Enstehen eines Rezidivs des Sinus pilonidalis entgegenwirkt. Es wurden in einem Zeitraum von 38 Monaten 42 Patienten nach dieser Methode operiert. Postoperative Wundheilungsstörungen oder Rezidive wurden nicht beobachtet. Im Vergleich der eigenen Ergebnisse mit der Literatur weist die Transpositionslappenplastik nach Limberg bessere Resultate auf als andere Verfahren mit offener Wundbehandlung, Primärnaht oder Z-Plastik und muß derzeit als die Operationsmethode der Wahl betrachtet werden.
