Zusammenfassung
Fragestellung: Abundante Blutungen nach Prostata- oder Harnblasenoperationen fordern im kritischen Stadium den Notfall-Behandler auch zu Überlegungen nach für den Patienten minimal belastenden Blutstillungsstrategien heraus. Die perkutane transvasale Embolisation (PTE) der internen Iliakaarterien bds. zeichnet sich hierbei durch eine außerordentliche Effektivität aus.Material und Methode: Bei 27 Patienten mit persistierender, schwer zu beherrschender Blutung nach Eingriffen im Prostata- oder Harnblasenbereich kam die PTE zum Einsatz.Ergebnisse: In 85 % der Fälle kam es zur prompten, anhaltenden Blutstillung.Schlussfolgerung: Die PTE bietet sich bei hohem Risikograd reoperativer Interventionen als geeignete, frühzeitig einzusetzende Alternativmaßnahme an.
Abstract
Purpose: Abundant hemorrhage after prostatic or bladder surgery requires that, in the critical phase, the emergency doctor should consider the use of hemostatic strategies which present as little stress as possible to the patient. In this connection the percutaneous transversal embolization (PTE) of the internal iliac arteries on both sides shows to be extremely efficient.Material and Methods: PTE was used on 27 patient with persistent, hardly controllable hemorrhage after surgery in the prostatic or bladder areas.Results: In 85 % of the cases the hemorrhage was promptly and permanently stopped.Conclusions: PTE seems to be an appropriate, early-to-use alternative measure for high-risk re-operative interventions.
