Malposition of the Lower Lateral Crus: Recognition and Treatment

ABSTRACT
-A review of the lateral crus malposition problem is made. The keys to diagnosis and the potential problems with this deformity are outlined. These include both aesthetic and functional (airway) problems. An analysis of the anatomy of the deformity is also made. Indications for treatment are reviewed as well. The treatment plan is then reviewed which include several options: (1) lateral crus rotation with graft (open approach), (2) rotation alone, (3) graft the rim only, and (4) complete excision of the lateral crus with immediate replacement. Indications and treatment details for each modality are given along with representative results, including when malrotation treatment is not undertaken.
