Antiepileptische Therapie nach einem ersten generalisierten tonisch-klonischen Anfall im Erwachsenenalter: Kontra

Zusammenfassung

Mit Ausnahme von akuten symptomatischen Anfällen, Hinweisen auf eine beginnende idiopathische Epilepsie oder einem initialen Status epilepticus besteht zur Zeit aus folgenden Gründen keine zwingende Therapieindikation nach einem ersten generalisierten tonisch-klonischen epileptischen Anfall (GTKA) im Erwachsenenalter: 1. Es gibt im Einzelfall keine verlässliche Möglichkeit, nach einem ersten GTKA vorherzusagen, ob sich eine behandlungsbedürftige Epilepsie entwickeln wird oder nicht. 2. Eine syndromspezifische Epilepsiediagnose (mit dadurch verbesserter Einschätzung des Spontan- und Therapieverlaufs) gelingt nach einem ersten GTKA oft nicht. 3. Ein Rezidivrisiko von etwa 50 % innerhalb der nächsten 2 - 3 Jahre bedeutet, dass eine antiepileptische Therapie bei etwa jedem zweiten Betroffenen überflüssig ist. 4. Mehrere prospektive Studien haben belegt, dass die Therapieprognose auf lange Sicht nach einem Behandlungsbeginn schon nach dem ersten Anfall nicht besser ist als nach dem zweiten. 5. Trotz Einleitung einer antiepileptischen Therapie nach einem ersten Anfall haben 25 - 50 % der Betroffenen innerhalb der nächsten zwei Jahre ein Anfallsrezidiv. Sogar bei Patienten mit neu diagnostizierten fokalen Anfällen konnte die bislang einzige randomisierte Studie nur einen marginalen Nutzen der medikamentösen Behandlung gegenüber Plazebo nachweisen. 6. Bei seltenen Anfällen treten bislang von Befürworten der Sofortbehandlung in den Vordergrund gestellte sozialmedizinische Konsequenzen (2. Anfall = Epilepsie mit z. B. Verlust der Fahrtauglichkeit) u. a. deswegen in den Hintergrund, weil so genannten Oligo-Epilepsien nach neuen Vorschlägen der Internationalen Liga gegen Epilepsie nicht mehr notwendigerweise zu den Epilepsien zählen.

Initiation of Phamacotherapy after a First Generalized Tonic-Clonic Seizure in Adults: Contra

With the exception of acute symptomatic seizures, hints to a developing idiopathic epilepsy or an initial status epilepticus there is at present in general no indication for the initiation of a therapy with antiepileptic drugs (AEDs) after a first generalized tonic-clonic seizure (GTCS) in adults for the following reasons: 1. For the individual patient with a first GTCS there is no reliable method to predict whether an epilepsy requiring AED treatment will develop or not. 2. A syndrome-specific epilepsy diagnosis (with corresponding improvement in knowledge of spontaneous and therapeutic prognosis) is often not possible after a first GTCS. 3. A recurrence risk of about 50 % for the following 2 - 3 years means that AEDs are not necessary in about every second patient. 4. Several prospective studies have demonstrated that the long-term therapy prognosis does not depend on whether treatment is started after the first or second seizure. 5. In spite of initiation of AED treatment after the first seizure 25 - 50 % of the patients will have recurrent seizures within the next 2 years. Even in patients with newly diagnosed focal seizures, the only randomized drug trial could only demonstrate marginal benefits in comparison to placebo. 6. In patients with only rare seizures the arguments regarding social consequences put forward by proponents of treatment after the first seizure (second seizure = epilepsy with e.g. loss of driving license) are becoming less important due to new proposals of the International League against Epilepsy regarding so-called oligo-epilepsy no longer necessarily as epilepsy.
