Internal Fixation of Proximal Humeral Fractures with a Blade-Plate

Abstract

Different methods of internal fixation have been used to treat fractures of the proximal humerus. The main complication generally is secondary displacement. We decided to use a blade-plate for the fixation of 2, 3, and 4 parts fractures and we present our experience of 26 cases between 1996 and 1998. The results are generally good. For the few cases of secondary displacement we have observed, we made a special study.
We concluded that this implant could decrease the indication of shoulder prosthesis, and also decrease the incidence of secondary displacement in comparison with conventional osteosynthesis devices.

Zusammenfassung

Zur Behandlung proximaler Humerusfrakturen werden unterschiedliche Methoden der Endofixation verwendet. Die Hauptkomplikation ist im Allgemeinen die sekundäre Dislozierung. Zur Fixation von 2, 3 und 4 Fragmentfrakturen wurde von uns eine „Blade-plate” gewählt und die Ergebnisse von 26 Fällen zwischen 1996 und 1998 evaluiert. Die Ergebnisse waren im Allgemeinen gut. In einer Extrastudie untersuchten wir die Ursachen der sekundären Fragmentdislozierung.
Wir schlussfolgern, dass die Verwendung dieses Implants die Indikationsrate für eine Schulterprothese sowie die Inzidenz einer sekundären Fragmentdislozierung zu senken vermag im Vergleich mit herkömmlichem Osteosynthesematerial.
