Abstract
Aims
Infertility in patients with posterior urethral valves (PUV) is known but the exact incidence and etiology is still speculative; it may be partly due to dysfunction of genital organs. The aim of this study is to analyse the hormonal profile of post-pubertal patients with PUV.
Materials and Methods
All post-pubertal patients with posterior urethral valves (age > 16 years) visiting the follow-up urology clinic of our department from 1985 to December 1999 were contacted and requested to participate in the study; 9 patients agreed and form the study group. The medical records were reviewed. Physical examination included examination of the genitalia and secondary sexual characteristics. Blood samples were taken for estimation of lutenizing hormone (LH), follicle-stimulating hormone (FSH), prolactin and testosterone.
Results
Ages ranged from 16 - 21 years (mean age = 17.5 years). Physical examination was normal for the entire study group except for 2 patients; one patient had hypertension and the other had unilateral undescended testis at the time of initial presentation. Secondary sexual characteristics were normal in all patients. At the time of the study, one patient had chronic renal failure; in the others the renal function tests were normal. LH and testosterone were normal in all patients including the patient with unilateral undescended testis, and one patient had marginally elevated FSH. Hyperprolactinemia was noted in 5 of 9 patients.
Conclusion
In the present study, hyperprolactinemia was seen in 62 % (5 of 9) patients. Hyperprolactinemia is known to cause slow ejaculation and can have an impact on the subsequent fertility status of these patients. The bearing of these findings on subsequent sexual and fertility functions of PUV patients requires further investigation.

L'infertilité chez les patients avec des valves de l'urètre postérieur (PUV) est connue mais l'incidence exacte et l'étiologie est encore non claire. Elle peut être due à une dysfonction partielle des organes génitaux. Le but de cette étude est d'analyser le profil hormonal post-pubère des patients avec PUV.
Matériel et Méthodes
Tous les patients post-pubères traités pour des valves de l'urètre postérieur (âge > 16 ans) suivis dans le département d'urologie de 1985 à 1999 étaient contactés et requis pour participer à l'étude; 9 patients étaient d'accord et forment le groupe d'étude. Les observations médicales étaient revues. Un examen clinique incluait l'examen des organes génitaux et des caractères sexuels secondaires. Des examens biologiques étaient réalisés pour apprécier l'hormone luténique (LH), la FSH, la prolactine et la testostérone.
Résultats
L'âge varie de 16 à 21 ans (âge moyen = 17.5). L'examen clinique était normal dans le groupe entier, à l'exception de 2 patients. Un des patients avait une hypertension et l'autre une ectopie testiculaire unilatérale au moment de l'examen. les caractères sexuels secondaires étaient normaux chez tous les patients. Au moment de l'examen, l'un des patients avait une insuffisance rénale chronique et chez les autres, la fonction rénale était normale. Le taux de LH et de testostérone était normal chez tous les patients, incluant le patient avec une ectopie testiculaire unilatérale, et un patient avait une élévation modérée de FSH. Une hyperprolactinémie était notée chez 5 des 9 patients.
Conclusion
Dans cette étude, l'hyperprolactinémie est vue chez 62 % (5/9) des patients. L'hyperprolactinémie est connue pour causer une éjaculation lente et peut avoir un impact sur la fertilité de ces patients. Les conséquences de ces découvertes sur les fonctions sexuelles et la fertilité de ces patients avec PUV nécessitent des investigations complémentaires.

Objetivo
La infertilidad es una consecuencia conocida de las válvulas de la uretra posterior (PUV) pero su etiología e incidencia exacta son desconocidas y puede ser debida en parte a disfunciones de los organos genitales. El objetivo de este estudio es analizar el perfil hormonal de pacientes postpuberales con PUV.
Material y Métodos
Todos los pacientes postpuberales con válvulas de uretra posterior (edad superior a 16 años) seguidos en la consulta de nuestro Departamento entre 1985 y 1999 fueron contactados para solicitarles su participación en el estudio. Aceptaron 9 pacientes que forman el grupo de estudio. Se revisaron las historias clínicas. El examen físico incluyó el examen de los genitales y de los caracteres sexuales secundarios. Se tomaron muestras para estimar la hormona luteinizante (LH), la hormona folículo- estimulante (FSH), la prolactina y la testosterona.
Resultados
La edad osciló entre 16 y 21 años (media, 17.5). El examen físico fue normal en todos excepto 2 pacientes: 1 tenía hipertensión y el otro maldescenso testicular unilateral en el momento de la presentación. Las características sexuales secundarias eran normales en todos los pacientes. En el momento del estudio uno sufría insuficiencia renal crónica y en los demás las pruebas de función eran normales. La LH y la testosterona eran normales en todos los pacientes incluyendo el que tenía un maldescenso unilateral y uno que tenía niveles de FSH marginalmente elevados. 5/9 pacientes tenían hiperprolactinemia.
Conclusión
En este estudio se vió hiperprolactinemia en 62 % (5/9) pacientes. Esta es una causa conocida de eyaculación retardada y puede tener un impacto en la fertilidad de estos pacientes. Las posibles consecuencias sobre la función sexual y la fertilidad de los pacientes con PUV requieren más investigación.

Zielsetzung
Unfruchtbarkeit ist bei Patienten mit hinteren Harnröhrenklappen ein bekanntes Phänomen, jedoch sind die Häufigkeit und Ätiologie unklar. Im allgemeinen wird eine generelle Dysfunktion der Genitalorgane angenommen. Ziel der vorliegenden Untersuchungen war es, hormonelle Profile von Patienten nach der Pubertät zu erstellen.
Methodik
Alle Patienten mit posterioren Harnröhrenklappen, die älter als 16 Jahre alt waren und die Pubertät hinter sich hatten, wurden nachuntersucht. Es handelt sich um Patienten, die zwischen 1985 und Dezember 1999 an unserer Klinik behandelt wurden. 9 Patienten waren mit einer Nachuntersuchung einverstanden. Die Krankendaten dieser Kinder wurden evaluiert und die Patienten klinisch im Hinblick auf ihre Genitalentwicklung untersucht. Darüber hinaus wurden serologische Untersuchungen des Luteinisierungshormones (LH), des follikelstimulierenden Hormones (FSH) sowie von Prolaktin und Testosteron durchgeführt.
Ergebnisse
Das Alter der Patienten schwankte zwischen 16 und 21 Jahren (Durchschnitt 17,5 Jahre). Bei 7 von 9 Patienten war die klinische Untersuchung unauffällig, ein Patient litt unter einem Bluthochdruck, ein anderer unter einem einseitigen Hodenhochstand. Die sekundären Geschlechtsmerkmale waren bei allen Patienten normal. Zum Zeitpunkt der Untersuchung wies ein Kind ein chronisches Nierenversagen auf, bei den anderen waren die Nierenfunktionstests normal. Die LH- und Testosteronwerte waren ebenfalls bei allen Kindern unauffällig, auch bei dem Knaben mit einseitigem Hodenhochstand. Ein Kind hatte marginal erhöhte FSH-Werte. Eine Hyperprolaktinämie wurde bei 5 der 9 Kinder beobachtet.
Schlussfolgerung
In der vorliegenden Studie wurde eine Hyperprolaktinämie bei 62 % (5 von 9 Kindern) registriert. Eine Hyperprolaktinämie kann jedoch Ursache einer verzögerten Ejakulation sein und hat somit einen Einfluss auf die Fertilität dieser Patienten. Die Auswirkung der erhobenen Befunde auf das Sexualleben und die spätere Fertilität muss jedoch abgewartet werden und bedarf weiterer Untersuchungen.
