Severe Autonomic Dysfunction in an 11-Year-Old Girl with Generalised Tetanus

Abstract

Tetanus can occur even after minor injuries, and proper immunisation status must be ascertained and documented in every surgical patient. Failure to do so can have dramatic consequences. An 11-year-old non-immunised girl was admitted to the intensive care unit with severe generalised tetanus 8 days following an open fracture of the right forearm. Although she was under deep sedation and received high doses of opiates while being paralysed and mechanically ventilated, she developed pronounced cardiovascular instability with tachycardia and periods of rapid changes between arterial hyper- and hypotension secondary to severe autonomic dysfunction. Her cardiovascular status only began to stabilise after 14 days under concomitant treatment with clonidine, magnesium sulphate and labetalol.

Résumé

Une fillette de 11 ans non vaccinée était admise à l'unité des soins intensifs avec un tétanos généralisé 8 jours après une fracture ouverte de l'avant-bras droit. Malgré une sédation profonde, de fortes doses d'opiacés, une relaxation et une ventilation artificielle, elle a développé une instabilité cardiovasculaire profonde avec tachycardie et variations tensionelles brutales dues à une dysfonction du système nerveux autonome. La situation cardiovasculaire s'est stabilisée à 14 jours et sous traitement simultané par Clonidine, Sulphate de magnésium et Labetalol.

Resumen

El tétanos puede ocurrir incluso después de heridas menores. Es imperativa una vacunación adecuada en todo paciente quirúrgico. Si esto no se hace pueden producirse dramáticas consecuencias. Una niña de 11 años no inmunizada fue ingresada en la Unidad de Cuidados Intensivos con tétanos generalizado grave 8 días después de una fractura abierta del antebrazo derecho. Aunque estaba bajo sedación profunda y recibió altas dosis de opiáceos, manteniéndose paralizada y ventilada desarrolló grave inestabilidad cardiovascular con taquicardia y periodos de cambios rápidos de la presión arterial secundarios a una grave disfunción autonómica. Su estado cardiovascular sólo comenzó a estabilizarse tras 14 días de tratamiento simultáneo con clonidina, sulfato de magnesio y labetalol.

Zusammenfassung

Ein 11-jähriges Mädchen wurde wegen eines generalisierten Tetanus auf die Intensivpflegestation aufgenommen. Sie hatte sich acht Tage zuvor eine offene Vorderarmfraktur zugezogen und war trotz fehlender Grundimmunisierung nicht geimpft worden. Trotz tiefer Sedation, hochdosierter Opiate, Muskelrelaxantien und mechanischer Beatmung entwickelte sich eine ausgeprägte kardiovaskuläre Instabilität mit Tachykardie und raschem Wechsel zwischen Perioden arterieller Hyper- und Hypotonie aufgrund einer schweren autonomen Dysfunktion. Die Kreislaufverhältnisse stabilisierten sich erst nach 14 Tagen allmählich unter gleichzeitigem Einsatz von Clonidin, Magnesiumsulfat und Labetalol.
