Conservative Management of Intrahepatic Perforation of the Gallbladder Secondary to Acalculous Cholecystitis

Abstract
Gallbladder (GB) perforation is a rare complication of acute acalculous cholecystitis. This complication mostly manifests as acute free perforation into the peritoneal cavity, subacute pericholecystic abscess, or chronic perforation with cholecystoenteric fistula. Perforation of the GB into the liver is extremely rare, and was reported only in adults, of whom all were treated surgically. The authors present an intrahepatic GB perforation secondary to acute acalculous cholecystitis, and its successful conservative management in a 13-year-old boy.
Résumé
La perforation vésiculaire est une complication rare de la cholécystite aiguë sans lithiase. Il s'agit d'une perforation aiguë avec déversement du contenu biliaire dans la cavité péritonéale, formation d'abcès péricholécystique aigu ou perforation chronique avec formation d'une fistule cholécystoentérique. La perforation de la vésicule biliaire est une pathologie très rare, généralement décrite chez l'adulte. Le traitement est chirurgical. Nous présentons un cas de perforation vésiculaire intrahépatique secondaire à une cholécystite sans lithiase chez un enfant de 13 ans. Le traitement conservateur était efficace.
Resumen
La perforación vesicular es una complicación rara de la colecistitis acalculosa. Se manifiesta generalmente como perforación aguda libre en la cavidad peritoneal, como absceso pericolecístico subagudo o como perforación crónica con fístula colecistoentérica. La perforación vesicular dentro del hígado es extremadamente rara y ha sido solamente publicada en adultos que siempre fueron tratados quirúrgicamente. Los autores presentan una perforación vesicular intrahepática secundaria a colecistitis acalculosa tratada conservadoramente con éxito en un chico de 13 años.
Zusammenfassung
Eine Gallenblasenperforation ist eine sehr seltene Komplikation einer akuten Cholezystitis ohne Gallensteine. Die Komplikation manifestiert sich meist in Form einer freien Perforation in die Bauchhöhle, eines subakuten, pericholezystitischen Abszesses oder einer chronischen Perforation mit cholezystoenteraler Fistel. Eine Perforation der Gallenblase in die Leber ist außerordentlich selten und wurde nur bei Erwachsenen beobachtet. Alle Patienten wurden chirurgisch behandelt.
Die Autoren stellen einen Fall einer intrahepatischen Gallenblasenperforation vor, die nach einer Cholezystitis ohne Gallensteinnachweis aufgetreten ist. Die schwere Erkrankung wurde erfolgreich konservativ behandelt, der Patient war ein 13 Jahre alter Knabe.
