Zusammenfassung
Asthma und Gastroösophagealer Reflux (GER) haben in der Gesamtbevölkerung eine hohe Prävalenz. Ihr gemeinsames Auftreten ist nach epidemiologischen Studien dennoch überzufällig häufig. Verschiedentlich wurde daher postuliert, dass GER Asthma auslöst oder verschlechtert. Andere Hypothesen gehen davon aus, dass Asthma GER auslöst oder verschlimmert oder anti-asthmatische Therapie GER verursacht. Die Literatur zu diesen praktisch relevanten Fragen ist uneinheitlich. Bis heute gibt keine überzeugenden Daten, die belegen, dass GER wesentliche Ursache oder gar Auslöser von Asthma bronchiale ist. Verschiedene Modelle, die den Einfluss von GER auf Asthma oder asthmatische Entzündung untersuchten, kommen zu uneinheitlichen oder widersprüchlichen Resultaten. Es ist unbewiesen, dass GER die endobronchiale Entzündung beim Asthma verstärkt. Interventionsstudien, die die Wirkung von GER-Therapie auf das Asthma untersuchten, registrieren vereinzelt eine symptomatische Besserung ohne klinisch relevante Lungenfunktionsverbesserungen. Ob Asthma GER auslöst oder verursacht, ist ebenfalls unklar. Obwohl einzelne anti-asthmatische Medikamente in hoher Dosierung GER auslösen oder verschlimmern können, kann dieser Effekt die Häufigkeit von GER beim Asthma vor dem Hintergrund moderner Asthmatherapie nicht erklären. Dennoch ist anzunehmen, dass GER Symptome auslösen oder verschlimmern kann, die mit Asthma assoziiert werden, ohne tatsächlich einer Bronchialobstruktion oder einer Asthma-Exazerbation zu entsprechen. Nachgewiesener, symptomatischer GER sollte aber auch oder gerade beim Asthma behandelt werden. Ziel ist die symptomatische und allenfalls in zweiter Linie eine lungenfunktionelle Verbesserung. Die Frage, ob sich Asthma durch GER-Therapie bessern lässt, muss in gut kontrollierten Langzeit-Studien weiter geprüft werden.
