Iron Deficiency Anemia in Children with Exstrophy of the Cloaca

Abstract

The clinical records of twenty-five children with exstrophy of the cloaca (EC) were retrospectively reviewed to evaluate the prevalence and the clinical characteristics of iron deficiency anemia (IDA). Five of the 25 children with EC (20 %) exhibited IDA at some point. Their ages at the time of diagnosis were between 1.9 and 13.0 years. In the four cases where IDA was thought to be related to iron malabsorption secondary to short-bowel syndrome, its treatment required longer periods of iron supplementation to correct the anemia and to restore the total body iron stores. Physicians caring for children with EC should monitor for and treat IDA as part of a comprehensive management plan.

Résumé

Une revue rétrospective des dossiers cliniques de 25 enfants ayant une exstrophie du cloaque est présentée afin d'évaluer la prévalence et les caractéristiques cliniques de l'anémie férriprive. Cinq des 25 enfants ayant une exstrophie du cloaque (20 p.100) avaient une anémie ferriprive. L'âge au moment du diagnostic de l'anémie était de 1,9 à 13 ans. Chez quatre patients, dont l'anémie ferriprive était attribuée à une malabsorption secondaire à un syndrome du grêle court, la période d'apport oral en fer nécessaire pour corriger l'anémie et restaurer les réserves corporelles était plus longue. Le dépistage et le traitement d'une anémie ferriprive doivent faire partie de la prise en charge globale des enfants présentant une exstrophie du cloaque.

Resumen

Hemos revisado retrospectivamente las historias de 25 niños con extrofia cloacal (EC) para evaluar la prevalencia y las características clínicas de la anemia ferropénica (IDA). Cinco de ellos (20 %) sufieron tal anemia en algún momento de su historia. Las edades de diagnóstico oscilaron entre 1.9 y 13 años. En los 4 casos en los que se pensó que la anemia era secundaria a malabsorción por síndrome de intestino corto fue necesaria una terapéutica marcial de larga duración para restablecer los depósitos de hierro. Los médicos que se encargan de tratar a estos pacientes deben tener presente la IDA considerándo su tratamiento como como una parte integrante del plan terapéutico de la EC.

Zusammenfassung

Die klinischen Daten von 25 Patienten mit Kloakenekstrophie wurden retrospektiv im Hinblick auf die klinischen Charakteristika einer Eisenmangelanämie untersucht. Fünf der 25 Kinder (20 %) zeigten eine Eisenmangelanämie. Das Alter zum Zeitpunkt der Diagnose schwankte zwischen 1,9 und 13 Jahren. In 4 Fällen war die Anämie korreliert mit einer Eisenmalabsorption als Folge eines Kurzdarmsyndroms. Die Therapie bestand in langfristiger Eisengabe. Die Arbeit wird vorgestellt, um Pädiater und Kinderchirurgen daran zu erinnern, dass bei Kindern mit einer Blasenekstrophie als Folge des vorliegenden Kurzdarmsyndroms eine Eisenmangelanämie nicht selten ist und die Fahndung nach dieser Begleiterkrankung zum Managementplan von Kindern mit Kurzdarmsyndrom, aber auch Blasenekstrophien, gehört.
