Abstract
TAR syndrome is manifested by the presence of hypomegakaryocytic thrombocytopenia and the bilateral absence of radii. An 8-day-old female newborn was referred to our clinic with diagnosis of TAR syndrome and symptoms of intestinal obstruction. On perineal examination she also had anal atresia with rectovestibular fistula. Because of persistent bile-stained drainage from the nasogastric tube, an upper gastrointestinal contrast study was performed and partial duodenal obstruction was detected. At operation, an annular pancreas was encountered and side-to-side duodenoduodenostomy was performed.
A small number of gastrointestinal system malformations associated with TAR syndrome has been reported. Our case is unique; the association with annular pancreas and anal atresia with rectovestibular fistula has never been previously reported.
Résumé
Le syndrome TAR est défini comme la présence d'une thrombopénie hypomégacaryocytique associée à une absence bilatérale de radius. Une fillette de 8 jours présentant une occlusion intestinale était adressée pour exploration d'un syndrome de TAR. L'examen clinique montrait la présence d'une atrésie de l'anus avec une fistule rectovestibulaire. Un transit gastroduodénal réalisé en raison de la présence de bile dans l'aspiration nasogastrique a mis en évidence une occlusion duodénale partielle. L'intervention chirurgicale pour duodenoduodenostomie latérolatérale a relevé un pancréas en anneau.
Quelques cas de syndrome de TAR associé aux malformations du système gastrointestinal sont rapportés dans la littérature. Notre cas est unique par l'association d'un pancréas en anneau avec une atrésie de l'anus et une fistule rectovestibulaire.
Resumen
El síndrome TAR se manifiesta por la presencia de trombocitopenia hipomegacariocítica y ausencia bilateral de radios. Hemos tratado una niña de 8 días con el diagnóstico de síndrome TAR y síntomas de obstrucción intestinal. El examen perineal mostró que tenía también atresia anal con fístula rectovestibular. El contenido bilioso del estómago condujo a un estudio radiológico con contraste que demostró obstrucción duodenal parcial. En la operación se encontró un páncreas anular y se realizó una duodeno-duodenostomía latero-lateral.
Se han publicado pocas anomalías gastrointestinales asociadas con el síndrome TAR. Nuestro caso es único ya que nunca había sido publicada antes la asociación con páncreas anular y atresia anal con fístula rectovestibular.
Zusammenfassung
Das TAR-Syndrom (Thrombozytopenie und fehlender Radius) ist gekennzeichnet durch eine hypomegakaryozytäre Thrombozytopenie und das beidseitige Fehlen des Radius. In der vorliegenden Arbeit wird ein 8 Tage altes, weibliches Neugeborenes vorgestellt, bei dem zusätzlich eine intestinale Obstruktion vorlag. Des Weiteren zeigte die perineale Untersuchung eine Analatresie mit rektovestibulärer Fistel. Darüber hinaus fand sich eine partielle Duodenalobstruktion, die sich klinisch in Gallenrückfluss aus der Magensonde äußerte. Bei der Operation fand sich ein Pancreas annulare, weshalb eine Duodenoduodenostomie erforderlich wurde.
Patienten mit TAR-Syndrom sind ungewöhnlich selten und in der Literatur kaum dargestellt. Im vorliegenden Fall ist das TAR-Syndrom in ungewöhnlicher Weise mit einem Pancreas annulare und einer Analatresie mit rektovestibulärer Fistel kombiniert, was bisher nicht berichtet wurde.
