Interventional Radiology Cases from the Royal Hospital for Sick Children, Glasgow: Percutaneous Metal Biliary Stent Placement for Palliation of Intra-Abdominal Desmoplastic Small Round-Cell Tumour in a Child

Abstract

Percutaneous placement of metal biliary stents for palliation of malignant processes affecting the porta hepatis is a well established technique in adults. We describe a case where the technique has been used successfully to treat obstructive jaundice in a ten-year-old boy suffering from intra-abdominal desmoplastic small round-cell tumour.

L'insertion percutanée d'une prothèse biliaire métallique pour le traitement palliatif d'une invasion maligne du porta hepatis est une technique bien codifiée chez l'adulte. Nous décrivons un cas où cette technique était utilisée avec succès chez un enfant de 10 ans présentant un ictère par obstruction due à une tumeur intra-abdominale desmoplasique à petites cellules.

Resumen

La colocación percutánea de stents biliares metálicos para paliar procesos malignos de la porta hepatis es un procedimiento bien establecido en adultos. Describimos un caso en el que esta técnica se usó con éxito para tratar una ictericia obstructiva en un niño de 10 años con tumor desmoplástico intra-abdominal de células redondas.

Zusammenfassung

Bei Erwachsenen ist die Einführung eines Metallstents in die Gallenwege zur palliativen Behandlung maligner Tumoren der Leberpforte ein bekanntes Verfahren. Im Kindesalter hingegen sind solche Techniken ungewöhnlich selten.

In der vorliegenden Arbeit wird der Fall eines malignen, abdominellen desmoplastischen Rundzelltumors beschrieben, der nicht operabel war. Palliativ wurde bei dem 10 Jahre alten Knaben eine intrahepatische Schienung der Gallenwege durchgeführt. Man sollte auch im Kindesalter an diese Möglichkeit denken.
