Zusammenfassung
Hintergrund und Fragestellung: Ein flächendeckendes Angebot von Schulungen für Typ-2-Diabetiker, wie im Rahmen der Disease-Management-Programme in Deutschland angestrebt, dürfte flächendeckend nur durch aktive Beteiligung von Hausarztpraxen möglich sein. Bisher bieten jedoch nur relativ wenige Praxen regelmäßig Schulungen an. Ziel unserer Untersuchung war es deshalb, von Hausärzten erkannte Motive und Hindernisse bei der Durchführung von Typ-2-Diabetikerschulungen zu erheben.Methode: Es wurden 18 leitfadengestützte Interviews mit Hausärzten durchgeführt, die mit Hilfe einer inhaltsanalytischen Strategie ausgewertet wurden. Motive und Hindernisse wurden dazu in Dimensionen und Unterkategorien eingeordnet.Ergebnisse: Ein hoher Stellenwert der Schulung in der Therapie, positive Effekte der Schulung in Gruppen sowie eine erhöhte Eigenverantwortung und Therapietreue beim Patienten wurden in allen Dimensionen als häufigste Motive angegeben. Als Hindernisse standen eine auf die Dauer nachlassende Patientenmotivation, die unzureichende finanzielle Honorierung sowie Zeitmangel im Vordergrund.Diskussion und Schlussfolgerungen: Aus den Ergebnissen der Interviews lassen sich verschiedene Ansatzpunkte ableiten, um die Bereitschaft von Hausärzten zur Durchführung von Typ-2-Diabetikerschulungen zu steigern. Neben einer attraktiveren Honorierung und einer Verbesserung der langfristigen Patientenmotivation könnten Kooperationen von mehreren Ärzten eine Chance bieten. Insbesondere durch so genannte Schulungsgemeinschaften lassen sich eventuell mehrere der in den Interviews häufiger genannten Hindernisse überwinden.
Abstract
Objectives: An area-wide implementation of the patient education for type-2-diabetics as intended within the scope of disease management-programmes in Germany is probably only possible through the active participation of general practices. So far, however, apparently only few practices offer regular patient education. Hence it was our aim to ask for the motives and obstacles of general practitioners as regards the implementation of patient education for type-2-diabetics.Methods: Based on a manual, 18 interviews with general practitioners were performed which were evaluated by means of content analysis. The motives and obstacles were then classified in dimensions and sub-categories.Results: A high significance of the patient education for the therapy, positive effects of the patient education in groups as well as an increased self-responsibility and compliance of the patient were the most frequently mentioned motives from all dimensions. Obstacles were the insufficiently sustaining patient motivation, the inadequate financial compensation as well as lack of time.Conclusions: Different approaches could be derived from the results of the interviews in order to increase the willingness of general practitioners to implement patient education for type-2-diabetics. Beside a more attractive reward and the improvement of a long-run patient motivation, co-operations between general practitioners could be of advantage. Particularly in so-called patient education associations, some of the more frequently mentioned obstacles could possibly be avoided.
