Abstract
Purpose: To evaluate whether prolonged postoperative stenting may reduce the risk of obstruction of the neourethra after TIP repair with deep glanular incision.
Methods: In a 1-year period 27 patients were operated for penile hypospadias using the TIP technique described by Snodgrass. In contrast to a previous study with 8 - 10 days of postoperative catheter drainage, the indwelling transurethral catheter was kept in place for 12 - 14 days. Deep incision of the urethral plate up to the tip of the glans is the most remarkable surgical detail, resulting in a meatus on top of the glans but in a defect on the dorsal rim of the neomeatus as well. After 3 - 6 months 22 patients were re-investigated during an outpatient visit using a scheme to describe the position of the neomeatus. Uroflowmetry was also performed. Information in three toddlers was gained by phone and 2 patients were lost to follow-up.
Results: Two patients returned with significant obstruction including an urethrocutaneous fistula in one. In contrast to the good assessment by parents and compared to the early appearance after catheter removal a change in meatal position was observed in the majority of patients. Only 6 patients presented with an unchanged meatal position whereas in 16 patients the meatus lost its oval or slit-like shape as well as its position on the tip of the glans. However, despite one obstructive meatus in the coronal position 15 patients showed a sufficient size and position of the meatus underneath the tip of the glans. Uroflowmetry revealed reduced peak-flow values (mean: 8.1 ml/s) in some of the 9 patients evaluated.
Conclusions: Our data indicate that prolonged stenting does not give better results in those TIP repairs, in whom the urethral plate was incised across the rim of the neomeatus. The early excellent aspect of the glans after stent removal is often impaired by partial closure of the glans incision with a short-term change in size and position of the meatus. To prevent this, the rim of the meatus during reconstruction should be kept completely epithelialised.

But: Evaluer si un calibrage post-opératoire prolongé peut réduire le risque d'obstruction du néo-urètre après TIP-réparation avec une profonde incision du gland.
Méthodes: Pendant une période de un an, 27 patients étaient opérés pour un hypospadias pénien en utilisant la technique TIP décrite par Snodgrass. Dans une première étude, le cathéter était laissé en place 8 à 10 jours; dans ce travail, le cathéter trans-urétral était laissé en place 12 à 14 jours. Une profonde incision de la plaque urétrale jusqu'à l'extrémité du gland est le détail technique le plus important, entraînant un méat au sommet du gland mais un défect sur la berge dorsale du néo-méat. Après 3 à 6 mois, 22 patients étaient examinés durant un séjour en hôpital de jour pour préciser la position du néo-méat. Une débitmétrie était réalisée. Pour deux patients perdus de vue, les informations étaient obtenues par téléphone.
Résultats: Deux patients se présentaient avec une obstruction significative incluant une fistule urétro-cutanée chez l'un. Par opposition aux bonnes impressions données par les parents et comparé à l'apparence précoce après l'ablation du cathéter, une modification de la position du méat était observée chez la majorité des patients. Seuls 6 patients se présentaient avec une position méatique inchangée alors que pour 16 le méat avait perdu son aspect et sa position au sommet du gland. Néanmoins, en dépit d'un méat sténosé en position coronale, 15 patients avaient une taille et une position du méat au-dessous du sommet du gland. La débitmétrie révélait une diminution du débit maximum (moyenne: 8,1 ml/s) chez les 9 patients où elle a pu être évaluée.
Conclusions: Notre étude indique qu'un cathétérisme prolongé n'améliore pas les résultats des TIP réparations pour lesquelles la plaque urétrale était incisée à travers la berge du néo-méat. L'excellent aspect du gland après l'ablation du cathéter est souvent altéré par une fermeture partielle de l'incision du gland peu de temps après. Pour prévenir ceci, la berge du méat durant la reconstruction doit être complètement épithélialisée.
Resumen
Objetivo: Evaluar si un sondaje prolongado puede reducir el riesgo de obstrucción en la neouretra tras reparación TIP con incisión glándica profunda.
En un periodo de un año operamos 27 pacientes por hipospadias peneano usando la técnica de TIP descrita por Snodgrass. En contraste con un estudio anterior en el que se empleó una sonda de drenaje durante 8 o 10 días se dejó éste en la uretra durante 12 a 14 días. La incisión profunda de la placa uretral hasta la punta del glande es el detalle quirúrgico más importante resultando en un meato apical pero también un defecto del borde dorsal del meato. Tras 3 a 6 meses fueron investigados de nuevo en la consulta 22 pacientes usando una rutina para describir la posición del neomeato. También se realizó uroflujometría. La información en 3 lactantes se obtuvo por teléfono mientras que 2 pacientes se perdieron de vista.
Resultados: Volvieron 2 pacientes con obstrucción significativa incluyendo en un caso una fístula uretro-cutánea. En contraste con la buena impresión dada por los padres y en comparación con la apariencia al retirar el cateter se observó un cambio en la posición meatal en la mayoría de los pacientes. Sólo 6 tenían una posición meatal invariable mientras que en 16 pacientes el meato perdió su forma oval o de hendidura así como su posición en la punta del glande. Sin embargo, a pesar de un meato obstructivo en posición coronal 15 pacientes tenían un tamaño suficiente y una posición del meato inmediatamente por debajo de la punta del glande. La uroflujometría reveló valores de flujo de pico bajo (media 8.1 ml/seg) en algunos de los 9 pacientes evaluados.
Conclusiones: Nuestros datos indican que un sondaje prolongado no da mejores resultados en esta reparación TIP en la que la placa uretral es incindida más allá del borde del neomeato. El excelente aspecto inicial del glande tras la retirada de la sonda es a menudo deteriorado por el cierre parcial de la incisión glándica con un cambio a corto plazo en el tamaño y posición del meato. Para prevenir éste debe mantenerse completamente epitelizado el borde del meato durante la reconstrucción.
Zusammenfassung
Zielstellung: Wir prüften prospektiv, ob die Rate postoperativer Obstruktionen nach TIP-Rekonstruktion der Harnröhre, bei der die Glans tief in der Mittellinie inzidiert wurde, durch längere Liegezeit einer transurethralen Harnableitung gesenkt werden kann.
Methodik: Wir verlängerten bei 27 in einem Einjahreszeitraum nach Snodgrass operierten Patienten die Liegezeit des transurethralen Katheters auf 12 - 14 Tage, nachdem zuvor in einem Einjahreszeitraum bei 13 Patienten eine 8- bis 10-tägige Schienung erfolgt war. Als wesentliches operatives Detail ist für alle Patienten die tiefe, bis in die Spitze der Glans reichende Inzision der Urethralplatte zu nennen, die einen Defekt auch im Bereich des Neomeatus hinterließ. Nach 3 - 6 Monaten stellten sich 22 der 27 Patienten ambulant zu einer Kontrolle vor. Bei 3 Kindern im Krabbelalter konnten telefonisch Informationen zum Verlauf eingeholt werden, 2 Patienten wurden nicht mehr erreicht.
Ergebnisse: Nach maximal 20 Monaten sind 2 relevante Obstruktionen und bei einem dieser Patienten eine Urethralfistel aufgetreten. Während die Bewertung durch die Patienteneltern in allen anderen Fällen gut war, wurde vom Nachuntersucher anhand eines Rasters eine Veränderung der ursprünglichen Meatusposition und -weite konstatiert. Während lediglich bei 6 Patienten die ursprüngliche, zum Zeitpunkt der Entlassung gesehene Position vorlag, war bei 15 Patienten der Meatus nicht mehr längsoval und weniger weit distal gelegen. Auch zeigten die bei den über dreijährigen Patienten im Follow-up durchgeführten Uroflows vorbehaltlich geringer Miktionsvolumina reduzierte Peak-Flow-Werte bei einigen der 9 untersuchten Patienten (Mittelwert: 8,1 ml/s). Die Befunde legen einen partiellen Verschluss des Neomeatus im Bereich der dorsalen Inzision der ehemaligen Urethralplatte nahe.
Schlussfolgerungen: Trotz zeitlich verlängerter Schienung der Neourethra konnten die Ergebnisse nach TIP-Rekonstruktionen, bei denen die Urethralplatte über den Neomeatus hinaus inzidiert wurde, nicht verbessert werden. Der zunächst exzellente Aspekt der Glans nach Entfernung des Katheters wird häufig durch einen partiellen Verschluss der dorsalen Glansinzision beeinträchtigt. Daraus resultiert eine Veränderung von Form und Position des Neomeatus. Um dies zu verhindern, sollte der dorsale Rand des Neomeatus bei der Rekonstruktion vollständig epithelisiert bleiben.
