Postcholecystectomy Syndrome with Special Regard to Children - A Review

Abstract
The incidence of abdominal symptoms after cholecystectomy in adults is high. Up to one third of the patients consult a doctor because of abdominal complaints within 1 year after the operation. In addition, a higher incidence of colon carcinoma after cholecystectomy has been reported in female patients. This article reviews the known facts on the “postcholecystectomy syndrome”. However, little is known about postcholecystectomy symptoms in children. Reports on cholecystectomy in children deal rather with feasibility than long-term outcome. Therapeutic concepts for children with symptomatic gallstone disease should consider the differences in the etiology of gallstone formation between children and adults. Therefore, we recommend a specific concept, including laparoscopic cholecystotomy, for children with temporary disorders causing gallstones, and laparoscopic cholecystectomy for all other patients. The impact of these procedures on postcholecystectomy symptoms in children and the impact of cholecystectomy during childhood on the incidence of right-sided colonic carcinoma remains to be determined.
Résumé
L'incidence des symptômes abdominaux après cholécystectomie chez l'adulte est important. Jusqu'à un tiers des patients consulte un médecin pendant la première année qui suit l'opération. Par ailleurs, une haute incidence du carcinome du côlon après cholécystectomie a été rapportée chez les femmes. Cet article revoit les faits connus concernant le «syndrome post-cholécystectomie». Néanmoins, peu de choses sont connues au sujet de ces symptômes post-cholécystectomie chez l'enfant. Les revues sur la cholécystectomie chez l'enfant discutent plutôt de la faisabilité que du devenir à long terme. Les concepts thérapeutiques pour les enfants avec une lithiase symptomatique doivent prendre en considération, dans la formation de la lithiase, une étiologie différente selon qu'il s'agit d'un enfant ou d'un adulte. Néanmoins, nous recommandons un concept spécifique incluant la cholécystostomie laparoscopique pour les enfants ayant des désordres entraînant l'apparition de lithiase et une cholécystectomie laparoscopique pour tous les autres patients. L'impact de ces procédés sur les symptômes post-cholécystectomie chez l'enfant et l'impact de la cholécystectomie durant l'enfance sur l'incidence du carcinome du côlon reste à déterminer.
Resumen
La incidencia de síntomas abdominales tras la colecistectomía es alta en adultos. Hasta un tercio de los pacientes consultan con el médico a causa de quejas abdominales un año después de la operación. Además ha sido publicada una incidencia más alta de carcinoma de colon tras colecistectomía en mujeres. Este artículo revisa los hechos conocidos del síndrome post-colecistectomía. Sin embargo, se sabe poco sobre dichos síntomas en niños. Las publicaciones sobre colecistectomía en niños se dirigen más bien a la factibilidad que a los resultados a largo plazo. Las actitudes terapéuticas para niños con enfermedad litiásica sintomática deben considerar las diferentes etiologías de la litiasis en niños y adultos. Por tanto, recomendamos colecistotomía laparoscópica en niños con enfermedades temporales que causan litiasis y colecistectomía laparoscópica para todos los demás. El impacto de estos procedimientos sobre los síntomas post-colecistectomía y sobre la incidencia de cáncer de colon derecho serán determinados en el futuro.
Zusammenfassung
Nach Cholezystektomie sind bei Erwachsenen häufig abdominale Beschwerden auffällig. Jeder dritte Patient sucht im ersten Jahr nach der Operation einen Arzt wegen abdominaler Symptome auf. Zudem ist bekannt, dass die Cholezystektomie bei weiblichen Patienten mit einer erhöhten Inzidenz von Kolonkarzinomen einhergeht. Dieser Artikel soll eine Übersicht über das „Postcholezystektomie-Syndrom“ geben. Wenige Daten liegen für Postcholezystektomie-Symptome bei Kindern vor. Bisherige Berichte zur Cholezystektomie waren auf technische Aspekte der Operation und weniger auf Langzeitergebnisse fokussiert. Therapeutische Konzepte für Kinder mit symptomatischem Gallensteinleiden sollten die Unterschiede in der Ätiologie der Gallensteinbildung zwischen Kindern und Erwachsenen berücksichtigen. Aus diesem Grund propagieren wir die laparoskopische Cholezystotomie für Patienten mit temporären Ursachen des Gallensteinleidens und die laparoskopische Cholezystektomie für alle übrigen Patienten. Zu klären bleibt der Einfluss dieser Eingriffe auf die Inzidenz von Postcholezystektomie-Symptomen bei Kindern und auf das Auftreten von Kolonkarzinomen im Langzeitverlauf.
