Sonographische Diagnose eines Leberabszesses infolge einer enterohepatischen Fistel bei einem Patienten mit Morbus Crohn

Zusammenfassung

Leberabszesse im Rahmen eines Morbus Crohn sind selten. In dieser Kasuistik wird über einen Patienten mit Morbus Crohn des oberen Gastrointestinaltraktes berichtet, bei dem es aufgrund einer Fistel vom Duodenum zum linken Leberlappen zur Ausbildung eines Leberabszesses kam. Unter einer Therapie mit Antibiotika und Infliximab® bildete sich der Abszess vollständig zurück. Auf eine perkutane Drainage wurde verzichtet, da sich in der echosignalverstärkten Sonographie zeigte, dass der Leberabszess von gut vaskularisierten und somit nicht eingeschmolzenen Leberarealen in mehrere fuchsbauartige Kammern unterteilt wurde. Die Arbeit belegt, dass der perkutan-konventionellen und der echosignalverstärkten Sonographie eine wesentliche Bedeutung in der Diagnostik von Leberabszessen bei Patienten mit Morbus Crohn zukommt. Dabei wird insbesondere gezeigt, dass auch enterohepatische Fisteln als Ursache eines Leberabszesses konventionell sonographisch exakt darstellbar sind.

Abstract

Liver abscesses in patients with Crohn’s disease are rare. We report on a patient with Crohn’s disease and a liver abscess of the left lobe caused by an enterohepatic fistula. With treatment of antibiotics and Infliximab® the abscess showed complete regression. Percutaneous drainage of the liver abscess was not performed because the abscess was shown not to be completely liquefied at echo-enhanced ultrasound. This case report demonstrates the usefulness of percutaneous conventional and echo-enhanced ultrasound for the diagnosis of liver abscesses. Furthermore, this case also shows that enterohepatic fistulas can be diagnosed precisely with percutaneous ultrasound.
