Effects of Whole-Body Hypoxic Preconditioning on Hypoxia/Reoxygenation-Induced Intestinal Injury in Newborn Rats

Abstract

Purpose: The precise cause of necrotizing enterocolitis (NEC) is elusive. Ischemia and reperfusion injury of the intestine has been considered to be a major contributing factor for NEC. Ischemic preconditioning is defined as one or more brief periods of ischemia with intermittent reperfusion that protects tissues against a sustained period of subsequent ischemia. Contribution of preconditioning to hypoxia/reoxygenation-induced intestinal injury in newborn rats has not been evaluated previously. Methods: The study was carried out on 1-day-old Wistar albino rat pups. Whole-body hypoxia and reoxygenation (H/R) was achieved by 10 min hypoxia using 95 % N₂ + 5 % CO₂ followed by 10 min reoxygenation with 100 % oxygen. Whole body hypoxic preconditioning (HP) cycles were performed with 3 min hypoxia and 5 min reoxygenation. Thirty-three pups were randomly allocated into 4 groups. Group 1 served as untreated controls. The pups in group 2 were subjected to H/R only. In groups 3 and 4, 1 cycle and 3 cycles of HP were performed prior to H/R, respectively. Animals were killed at the end of the protocols. Intestine specimens were obtained to determine the histological changes, as well as to measure the tissue malondialdehyde (MDA) and nitric oxide (NO) levels, and xanthine oxidase (XO) and myeloperoxidase (MPO) activities. Results: The microscopic lesions in H/R rat pups were virtually the same as those seen in neonatal NEC, with severe destruction of villi and crypts, in some cases extending to the muscularis. In both HP groups, the lesions were found to be milder. H/R resulted in a marked elevation in MDA and NO levels, and XO and MPO activities compared to the untreated controls. Both 1 cycle and 3 cycles of HP prior to H/R resulted in an obvious decrease in all biochemical parameters. Differences of the biochemical results between both HP groups were not statistically significant. Conclusion: This study revealed that whole-body hypoxic preconditioning is beneficial for hypoxia/reoxygenation-induced intestinal injury in newborn rats.

But: La cause précise de l'entérocolite nécrosante (NEC) est encore floue. L'ischémie et l'agression dû à la reperfusion de l'intestin sont considérés comme étant un des facteurs majeurs responsables de NEC. Le pré-conditionnement ischémique est défini comme un ou plusieurs bref(s) épisode(s) d'ischémie avec une reperfusion intermittente qui protège les tissus contre une période plus prolongée d'ischémie. Le pré-conditionnement au traumatisme hypoxie/réoxygénation n'a jamais été étudié au préalable chez le rat nouveau-né. Méthodes: L'étude a été réalisée sur des rats albinos Wistar de 1 jour de vie. L'hypoxie de l'ensemble du corps et la réoxygénation (H/R) étaient réalisés par 10 minutes d'hypoxie en utilisant un mélange 95 % de N₂ + 5 % de CO₂ suivies par 10 minutes de réoxygénation sous 100 % d'oxygène. Les cycles de pré-conditionnement (HP) étaient réalisés par 3 minutes de réoxygénation. 32 rats étaient divisés en 4 groupes. Le groupe 1 était le groupe contrôle non traité, le groupe 2 était soumis à H/R seulement. Dans les groupes 3 et 4, 1 cycle et 3 cycles de HP étaient réalisés avant H/R. Les animaux étaient tués à la fin du protocole. Des biopsies intestinales étaient obtenues pour déterminer les modifications histologiques ainsi qu'utilisées pour mesurer la malondialdehyde (MDA), le niveau d'oxyde nitrique (NO), de xanthine oxydase (XO) et de myélopéroxydase (MPO). Résultats: Les lésions microscopiques dans le groupe H/R étaient les mêmes que celles vues en cas d'entérocolite nécrosante néonatale avec une destruction sévère des villosités des cryptes, dans quelques cas étendues à la musculaire. Dans les deux groupes HP, les lésions étaient retrouvées moins importantes. H/R entraînait une élévation marquée de MDA et de NO, et des activités XO et MPO comparées au groupe non traité. Les deux types 1 ou 3 cycles de HP avant H/R entraînaient une diminution importante des paramètres biochimiques. Les différences des résultats biochimiques dans les deux groupes HP n'étaient pas significatifs. Conclusion: Cette étude révèle que le pré-conditionnement total est bénéfique sur les lésions d'hypoxie/réoxygénation chez le nouveau-né de rat.

Resumen

Objetivos: La causa exacta de la enterocolitis necrotizante (NEC) es desconocida. Las lesiones de isquemia/reperfusión del intestino han sido consideradas como factores contribuyentes mayores. Se define el precondicionamiento isquémico como uno o más periodos breves de isquemia con reperfusión intermitente que protegen los tejidos contra un periodo subsiguiente de isquemia prolongada. La contribución del precondicionamiento a las lesiones intestinales inducidas por hipoxia/reoxigenación en ratas recién nacidas nunca ha sido estudiada anteriormente. Métodos: El estudio se realizó con ratas Wistar albinas de 1 día de edad. Se consiguió una hipoxia y reoxigenación (H/R) corporal total de 10 minutos usando N₂ al 95 % + CO₂ al 5 % seguida de 10 minutos de reoxigenación con oxígeno al 100 %. Los ciclos de precondicionamiento hipóxico (HP) se consiguieron con 3 minutos de reoxigenación. Se dividieron randomizadamente 33 ratas en 4 grupos. El grupo 1 sirvió como control sin tratamiento. El grupo 2 recibió H/R solamente. En los grupos 3 y 4 se suministraron 1 y 3 ciclos de HP antes de la H/R respectivamente. Los animales fueron sacrificados al final del protocolo. Se obtuvieron muestras de intestino para determinar los cambios histológicos así como para medir las actividades de malon-dialdehido (MDA), óxido nítrico (NO), xantino-oxidasa (XO) y mielo-peroxidasa (MPO) tisulares. Resultados: Las lesiones microscópicas en ratas con H/R fueron virtualmente las mismas que las que se ven en la NEC neonatal con destrucción severa de los villi y de las criptas que en algún caso se extendían hasta la muscular. En ambos grupos HP las lesiones eran más leves. La H/R causó una elevación marcada de MDA, NO, XO y MPO en comparación con controles sin tratamiento. Tanto 1 como 3 ciclos de HP antes de H/R causaron una disminución obvia de todos los parámetros bioquímicos. Las diferencias de los resultados bioquímicos entre ambos grupos HP no fueron estadísticamente significativas. Conclusión: Este estudio revela que el precondicionamiento hipóxico corporal total es beneficioso sobre la lesión intestinal de H/R en ratas recién nacidas.

Zusammenfassung

Zielsetzung: Die genaue Ursache der nekrotisierenden Enterokolitis ist nicht bekannt. Ischämie und Reperfusionsschäden der Darmwand wurden als Hauptfaktor betrachtet. Die ischämischen Vorbedingungen sind definiert als mehr oder weniger kurze Perioden von Durchblutungsstörungen, die von intermittierender Reperfusion unterbrochen werden und das Gewebe gegen eine konsequente, anhaltende Ischämie schützen könnten. Dieser Wechsel von Hypoxie und Reoxygenierung des intestinalen Gewebes wurde bisher nicht näher untersucht und wird in der folgenden Arbeit an neugeborenen Ratten überprüft. Material und Methodik: Bei ein Tag alten Wistar-Albinorattenbabys wurde eine vollständige Hypoxie/Reoxygenierung (H/R) durchgeführt. Dabei wurde eine 10-minütige Hypoxie mit 25 %igem N₂ und 5 %igem CO₂ durchgeführt. Diese Periode war gefolgt von einer 10-minütigen Reoxygenierung mit 100 %igem Sauerstoff. Die Gesamtkörper-Hypoxiezyklen betrugen 3 Minuten, die Reoxygenierungszeit 5 Minuten. 33 Baby-Ratten wurden in 4 Gruppen eingeteilt. Die Gruppe 1 entsprach einer nicht behandelten Kontrollgruppe, die Gruppe 2 wurde einer einzigen Hypoxie und Reoxygenierung, die Gruppen 3 und 4 wurden einem Zyklus und 3 weiteren Zyklen vor einer Hypoxie und Reoxygenierung unterzogen. Die Tiere wurden am Ende des Protokolls sakrifiziert. Das entnommene Dünndarmgewebe wurde histologisch untersucht und die Malondialdehyde (MDA)- und die Stickstoffmonoxid (NO)-Spiegel sowie die Aktivität der Xanthinoxydase (XO) und der Myeloperoxidase (MPO) gemessen. Ergebnisse: Die mikroskopischen Ergebnisveränderungen bei den Hypoxie/Reoxygenierungsratten waren die gleichen wie bei der neugeborenen, nekrotisierenden Enterokolitis mit schwerer Zerstörung der Villi und Krypten, die in manchen Fällen bis in die Muskularis hineinreichten. Bei beiden Hypoxie- und Präkonditionierungsmodalitäten (HP-Gruppen) waren die Defekte jedoch leichter. Hypoxie und Reoxygenierung führten zu deutlich erhöhten MDA- und NO-Spiegeln sowie einer gesteigerten XO- und MPO-Aktivität im Vergleich zu Kontrolltieren. Sowohl die einzyklische wie die dreizyklische Präkonditionierung von Hypoxieintervallen vor Hypoxie und Reoxygenierung führten zu einem deutlichen Absinken aller biochemischen Parameter. Die Unterschiede der biochemischen Ergebnisse zwischen den HP-Gruppen waren jedoch nicht statistisch signifikant. Schlussfolgerung: Die Untersuchung zeigt, dass eine Gesamtkörperhypoxie-Präkonditionierung nützlich ist im Hinblick auf die Hypoxie/Reoxygenierung intestinaler Schäden bei neugeborenen Ratten.
