Zusammenfassung
Auch wenn bei der Parkinson-Erkrankung die motorischen Symptome im Vordergund stehen, tritt in fast der Hälfte der Fälle gleichzeitig eine Depression auf, die in der Behandlung berücksichtigt werden muss. Noch ist unklar, ob die Entwicklung einer depressiven Symptomatik reaktiv auf die Beeinträchtigung der Lebensqualität durch die Parkinson-Erkrankung auftritt oder auf eine gemeinsame Störung im Transmitterhaushalt zurückgeht. Neuere Medikamente, wie der COMT-Hemmstoff Entacapon, verbessern sowohl die motorischen als auch die psychischen Symptome der Parkinson-Erkrankung. Für das Kombinationspräparat Stalevo® mit Levodopa, Carbidopa und Entacapon wurde eine deutliche Reduktion von Wearing-off-Symptomen und eine Verkürzung der Off-Zeit nachgewiesen. In dem vorliegenden Fallbeispiel zeigte sich nach Umstellung auf Stalveo® und Absetzen des Dopaminagonisten eine beeindruckende Verbesserung des Allgemeinzustandes bei der Patientin, die neben der Parkinson-Erkrankung auch an einer rezidivierenden Depression litt. Die durchschnittliche tägliche Off-Zeit reduzierte sich von 120 Minuten auf Null und die Lebensqualität wurde deutlich verbessert. Es trat eine Stabilisierung der motorischen und psychischen Symptomatik ein.
Abstract
Even though motor symptoms are most prominent in Parkinson's disease, depression is seen in almost half of all patients and must be taken into account in the treatment. It remains unclear, whether the development of depressive symptoms is a reaction to the impairment in quality of life or is due to a common malfunction in transmitter metabolism. New drugs, like the COMT-inhibitor entacapone, may be able to improve motor symptoms as well as psychiatric symptoms in Parkinson's disease. For the combination tablet Stalevo® with levodopa, carbidopa and entacapone a significant reduction of symptoms of wearing-off and a shortening of off-time has been proven. In this case report the change of medication to Stalevo® lead to an impressive improvement of the general condition in a patient who suffered from Parkinson's disease and recurrent depressive episodes. Mean daily off-time was reduced from 120 minutes to zero and quality of life was clearly improved. A stabilisation of motor and psychiatric symptoms was seen.
