Die komplexe Pathogenese des Typ-2-Diabetes-mellitus

Zusammenfassung

Typ-2-Diabetes entsteht durch das Versagen der β-Zellen, die Insulinresistenz von Leber, Muskel und Fettgewebe durch eine erhöhte Insulinsekretion zu kompensieren. Die Diabetesentstehung ist zu einem hohen Anteil durch eine genetische Grundlage geprägt: Genvarianten, die für Insulinresistenz - meist assoziiert mit Adipositas - prädisponieren, treffen vermutlich mit diabetogenen Allelen, die allein für Inselzellversagen verantwortlich sind, zusammen. Das Diabetesrisiko wird außer durch die genetische Grundlage auch durch exogene Parameter modifiziert, unter denen das Ernährungsmuster eine zentrale Rolle spielt. Tierexperimentelle Daten zeigen, dass sowohl der Fett- (Lipotoxizität) als auch der Kohlenhydratanteil (Glukotoxizität) des Futters eine kausale Rolle in der Entstehung des Typ-2-Diabetes spielen. In humanen, prospektiven Kohortenstudien findet sich eine Modifikation des Diabetesrisikos durch die folgenden Faktoren: die Fettmasse des Körpers, der Anteil an gesättigten und trans-Fettsäuren in der Diät, der Ballaststoffaufnahme sowie der Qualität der aufgenommenen Kohlenhydrate (glykämische Last). Letzterer Effekt ist konsistent mit der Schlussfolgerung, dass das Diabetesrisiko durch erhöhte postprandiale Blutzuckerauslenkungen erhöht wird. Die Schlussfolgerung wird zudem durch die risikosenkende Wirkung einer Behandlung mit Acarbose oder Metformin gestützt. Der Einfluss der verschiedenen exogenen Faktoren ist verschieden groß: Eine Reduktion des zu hohen Körpergewichts um nur 4 kg reduziert das Diabetesrisiko bereits um ca. 50 %, während sich durch eine Verminderung der glykämischen Last auf die niedrigste Quintile nur eine Senkung um ca. 25 % erzielen lässt. Aus diesen Daten lässt sich schließen, dass die Qualität der verzehrten Kohlenhydrate - vermutlich über die Höhe der postprandialen Glukoseauslenkungen - einen Einfluss auf die Diabetesentstehung hat. Dieser Effekt tritt aber quantitativ hinter dem des Körperfetts zurück, sodass durch Gewichtsnormalisierung ein erheblich größerer präventiver Effekt erreichbar ist.

Abstract

Type-2 diabetes is the result of insulin resistance, usually associated with obesity, and a progredient β-cell failure. It is generally believed that β-cell dysfunction is caused by external factors, and three different noxious agents have been proposed: elevated blood glucose levels („glucotoxicity”), fatty acids and/or triglycerides accumulating in the β-cells („lipotoxicity”), and inflammatory cytokines from adipose tissue. This review summarizes the available evidence for the glucotoxicity concept, in particular with regard to the role of dietary carbohydrates. The data are consistent with the hypothesis that postprandial glucose excursions are a major factor in the development of type-2 diabetes. However, a dietary intervention reducing the glycemic load is probably less effective in reducing the diabetes risk than a reduction of body fat.
